摘要
[目的]观察辨证分型治疗老年尿路感染疗效。[方法]使用随机平行对照方法,将86例门诊及住院患者按掷骰子法简单随机分为两组。对照组43例左氧氟沙星,0.5g/次,1次/d。治疗组43例辨证分型,肝肾阴虚-大补阴丸合导赤散(甘草3g,龟板、知母各10g,熟地黄、黄柏、生地黄各15g、淡竹叶12g);脾肾两虚-大补元煎(党参12g,山药、熟地各15g,杜仲15g,鹿角胶12g,升麻5g);脾肾阳虚-金匮肾气丸(附子5g,山茱萸、白茯苓、丹皮、山药、车前子、泽泻、肉桂各10g,熟地黄、川牛膝各15g,升麻5g,白术炒18g;尿频尿痛加马齿苋30g,丹皮、车前草、黄芩、黄柏、石苇、瞿麦、白花蛇舌草各15g;血尿加小蓟、白茅各15g);1剂/d,水煎200m L,早晚口服。连续治疗28d为1疗程。观测临床症状、尿液、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈24例,显效10例,有效6例,无效3例,总有效率93.02%。对照组痊愈20例,显效9例,有效4例,无效10例,总有效率76.74%。治疗组疗效优于对照组(P<0.05)。[结论]辨证分型治疗老年尿路感染疗效满意,无严重不良反应,值得推广。
[Objective]To observe the Differential Diagnosis and Treatment of urinary tract infections in elderly effect.[Methods]Random parallel control method,86 cases of outpatient and inpatient dice law by simple random into two groups.The control group of 43 patients levofloxacin,0.5g/times,1 times/d.Treatment group of 43 patients Syndromes,liver and kidney-Dabuyin Pills Daochi Powder（Gancao 3g,Guiban,Zhimn 10g,Shudihaung, Huangbai,Shengdihaung 15g,Danzhuye 12g）;spleen deficiency-Da Bu Yuan Jian（Dangshen 12g,Sahnyao,Shudi 15g,Duzhong 15g,Lujiaojiao 12g,Shengma 5g）;spleen deficieney-Jingguishenqi pill（Fuzi 5g,Shanzhuyu,B aifuling,Danpi,Shanyao,Cheqianzi,Zexie,Rougui 10g,Shudihaung,Chuanniuxi 15g,Shengma 5g,Baizhu fry 18g;frequent,painful urination plusMaeixian 30g,Danpi,Cheqiancao,Huangqin,Huangbai,Shiwei,Qumai,Baihua sheshecao each 15g;hematuria Canada Xiaoji ,Baimao each 15g. 1 agent/d,decoction 200mL,sooner or later oral therapy 28d for a course of continuous observation of clinical symptoms,adverse effects of continuous therapy..2 courses,to determine efficacy.[Results]The cure 24 cases,10 cases markedly effective in 6 cases,3 cases,the total efficiency of 93.02%.The control group cured 20 eases,effective in 9 cases,effective in 5 cases,9 cases,the total efficiency of 79.07%.Treatment group than the control group（P〈0.05）.[Conclusion]Differential diagnosis and treatment of urinary tract infections in elderly results were satisfactory,no side effects,worthy of promotion.
