摘要
目的运用MRI分析腰椎后路Dynesys动态稳定系统对固定临近节段椎间盘的影响。方法将36例腰椎疾病患者随机分为Dynesys动态固定组(A组,12例)和融合内固定组(B组,24例)。手术前后均行腰椎常规MRI扫描,采用Oswestry功能障碍指数(ODI)评分进行临床功能评价,Woodend分级和表观弥散系数(ADC)评估腰椎间盘退变情况。结果与术前相比,A组和B组术后ODI评分均改善(P<0.05),A组末次随访时ADC值增加,而B组ADC值减少(P<0.05)。结论从MRI影像学角度分析,腰椎后路Dynesys动态稳定系统可预防和延缓动态固定节段的椎间盘退变。
Objective To analyze the effect of Dynesys dynamic stabilization system on the adjacent‐segment intervertebral discs by MRI technique .Methods A total of 36 patients with lumbar diseases underwent MRI scanning was randomly divided into two groups of A(treated with Dynesys dynamic stabilization system ,12 cases) and B (treated with spinal fusion and internal fixation ,24 cases) .The clinical function was evaluated by Oswestry disability index(ODI) score ,and lumbar disc degeneration was assessed by Woodend classification and apparent diffusion coefficient(ADC) score of MRI .Results Compared with before ,ODI scores were improved after operation in both groups (P〈0 .05) and ADC value at the time of final follow‐up was increased in group A(P〈0 .05) ,which was decreased in group B (P〈0 .05) .Conclusion MRI analysis indicates that Dynesys dynamic stabilization system of posterior lumbar can prevent and delay the intervertebral disc degeneration of fixed segments .
