摘要
目的研究临床药师对乳腺手术患者围术期抗菌药物干预的效果,评价临床药师在围术期抗菌药物合理性使用中的干预作用。方法采用回顾性调查方法,随机抽取2010-2011年167例(非干预组)及2012年158例(干预组)乳腺手术患者,对抗菌药物使用的合理性进行评价,应用SPSS 15.0进行统计分析。结果经临床药师干预,乳腺手术患者围术期预防性应用抗菌药物在预防使用率、药物选择、联合用药、用药时机与疗程等方面较干预前均有明显改善,抗菌药物预防使用率由干预前的93.41%下降至干预后10.76%;非干预组术前30min^2h或麻醉开始时给药分布率为3.85%,干预后提高至11.76%;非干预组抗菌药物预防使用品种主要以β-内酰胺酶抑制剂为主,其次为选用单环β-内酰胺类抗菌药物;非干预组中有84例存在联合使用抗菌药物,全部为二联用药,干预后联合用药为0。结论经过抗菌药物专项整治,抗菌药物不合理应用的现象得到显著改善,但在药物选择、使用时间等方面仍需要继续改进。
OBJECTIVE To study the effect of the intervention by clinical pharmacists on antibiotic usage during perioperative period for patients received breast surgery so as to evaluate the interventional functions .METHODS By means of retrospective investigation ,167 cases in 2010~2011 （non‐intervention group） and 158 cases in 2012 （intervention group） were selected randomly from records of breast surgery .Then the rationality of the periopera‐tive use of antibiotics was evaluated .SPSS 15 .0 was adopted for statistical analysis .RESULTS By the intervention of clinical pharmacists ,the application of perioperative prophylactic antibiotics in breast surgery was improved sig‐nificantly in respects of utilization rates ,medicine selection ,drug combination ,administration time and treatment course .Utilization rates of prevention decreased from 93 .41% to 10 .76% .Dosage rate （0 .5~2 h before surgery or anesthesia） was 3 .85% before the intervention （non‐intervention group） ,but increasing to 11 .76% after the intervention .β‐lactamase was chosen as the main antibiotics in the non‐infection group ,second by monocyclicβ‐lactam .In the non‐infection group ,84 cases received double combinations of antibiotics .After interventions ,no combined antibiotics were adopted .CONCLUSIONS The irrational application of antibiotics was significantly amel‐iorated by the special rectification of clinical antibiotic application ,yet it remains to be improved in drug selection and use time .
