摘要
目的 分析292例胎儿超声软指标（USM）的临床结局。方法 选取2010年1月~2013年12月在该院行介入性产前诊断USM异常孕妇292例为研究对象,统计其临床资料并进行回顾性分析。结果 检出染色体异常30例（10.27%）,包括21-三体5例,18-三体5例,13-三体1例。单纯USM异常其染色体异常检出率为4.73%,USM异常合并其他异常其检出率为17.89%。30例染色体异常者中18例引产,1例失访,其他随访均正常。262例染色体核型正常者中引产36例,失访24例,早产5例,足月分娩后发现异常者7例。结论 USM本身可能增加染色体异常风险,若合并其他异常更应重视并评估介入性产前诊断的必要性;对于染色体核型正常者也不能放松警惕,其后期可能出现其他异常,应加强围产期监护。
Objective To analyze the clinical outcome of ultrasonic soft marker （USM） of 292 fetuses. Methods A total of 292 pregnant women with abnormal USM results detected by invasive prenatal examination in the hospital from January 2010 to December 2013 were selected as study objects, the clinical data was recorded statistically and analyzed retrospectively. Results Thirty cases （ 10. 27% ） were diagnosed as chromosomal abnormalities, including five cases with trisomy 21, five cases with trisomy 18, and one case with trisomy 13. Among the cases with simple USM abnormality, the detection rate of chromosomal abnormalities was 4. 73%, the detection rate of abnor- mal USM combined with other abnormalities was 17. 89%. Among thirty cases with chromosomal abnormalities, 18 cases received induced a- bortion, and one case was lost to follow-up, the follow-up results of the other cases were normal. Among 262 cases with normal chromosomal karyotypes, 36 cases received induced abortion, 24 cases were lost to follow-up, 5 cases gave birth to premature infants, among the cases giving birth to mature infants, 7 cases were found with abnormal chromosomal karyotypes. Conclusion USM can enhance the risk of chro- mosomal abnormalities, more attention should be paid to when the cases combining with other abnormalities and the necessity of invasive pre- natal diagnosis should be evaluated; the women with normal chromosomal karyotypes also need more attention, perinatal monitoring should be enhanced to avoid other abnormalities at the later stage.
