摘要
目的探讨盆底功能锻炼疗法(PFME)与生物反馈-电刺激-盆底功能锻炼联合疗法对产后妇女性生活质量的作用。方法选择2013年6月-2014年9月于重庆医科大学附属第二医院分娩的88例产妇作为研究对象,将其随机分为生物反馈-电刺激-盆底肌肉锻炼联合疗法治疗组(45例)和PFME组(43例)。于产后5d,42d,3个月随访,随访过程中医师评估2组产妇的阴道壁膨出情况,采用生物反馈-电刺激康复治疗仪检测盆底肌电压、盆底肌力并让产妇填写《女性性功能指数评分量表》(FSFI)以评估治疗后的性生活质量。结果2组产妇产后3个月阴道前、后壁膨出人数所占比例均较治疗前明显降低,差异有统计学意义(P<0.05);2组产妇产后42d的盆底肌电压均高于产后5d,差异有统计学意义(P<0.05);2组产妇产后42d盆底肌力级别(Ⅰ类纤维及Ⅱ类纤维)均高于产后5d,差异有统计学意义(P<0.05);2组产妇产后3个月盆底肌电压及盆底肌力级别均高于产后5d,差异有统计学意义(P<0.05)。联合治疗组产妇产后3个月盆底肌电压及盆底肌力级别均高于PFME组,差异有统计学意义(P<0.05);联合治疗组与PFME组产妇产后42d内恢复性生活的比例分别是40.00%,34.88%,3个月内恢复性生活的比例分别是84.44%,79.07%,但2组差异均无统计学意义;2组产妇产后42dFSFI数值比较,差异无统计学意义;产后3个月联合治疗组产妇性欲、性高潮、性满意度、性疼痛数值及总分较PFME组明显升高,差异有统计学意义(P<0.05)。结论生物反馈-电刺激-盆底肌肉锻炼联合疗法不仅对产后妇女的盆底功能及性生活质量有改善作用,还为产后妇女性功能障碍的治疗提供了临床依据。
Objective To investigate the function of pelvic floor muscle exercise (PFME) and the combined therapy of biofeedback-electrical stimulation-pelvic floor muscle exercise on the quality of sexual life in postpar- tum women. Method 88 cases of lying-in women received in the second Affiliated Hospital of Chongqing Medical University from June 2013 to September 2014 were randomly divided into a combination therapy group (45 cases) and a PFME group (43 cases). In follow-ups of 5 days, 42 days and 3 months after their delivery, the physician assessed maternal vaginal wall bulge, detected their pelvic floor muscle voltage and pelvic floor muscle strength by using biofeedback-electrical stimulation rehabilitation instrument and asked these women to fill out the "Female Sexual Function Index Rating Scale" (FSFI). Results After the treatment, the number of women with anterior and posterior vaginal wall bulge in postpartum 3 months was obviously lower than before the treatment in both groups and the difference was statistically significant (P〈0.05) ; the pelvic floor muscle voltage of postpartum 42 days in the two groups was higher than postpartum 5 days and the difference was sta- tistically significant (P〈0.05) ; the pelvic floor muscle strength levels of postpartum 42 clays in two groups ( I and II types of fiber) were higher than postpartum 5 days and the difference was statistically significant (P〈0.05) ; in 3 months postpartum, pelvic floor muscle voltage and pelvic floor muscle strength level in both groups were higher than postpartum 5 days and the difference was statistically significant (P〈0.05); but 3- month postpartum pelvic floor muscle voltage and pelvic floor muscle strength levels of the combination groupwere higher than those of the PFME group and the difference was statistically significant (P〈0.05) ; the rates of sexual life in postpartum 42 days were respectively 40.00% and 34.88%, the rates of sexual life in postpar- tum 3 months were 84.44% and 79.07%, but there was no significant difference between the two groups; the difference of FSFI in postpartum 42 days was not statistically significant in both groups; in postpartum 3 months, sexual desire, orgasm, sexual satisfaction and sexual pain of the combination group were significantly higher than the PFME group and the difference was statistically significant (P〈0.05). Conclusion The thera- py of biofeedback-electrical stimulation-pelvic floor muscle exercises can improve the function of pelvic floor and the quality of sexual life in postpartum women and it can also provide a clinical evidence for the treatment of postpartum sexual dysfunction.
