摘要
【目的】探讨经阴道“削苹果”式取子宫标本在腹腔镜巨大子宫全切术中的应用。【方法】收集腹腔镜巨大子宫全切术患者66例,依据患者意愿进行分组:经阴道“削苹果”取出子宫标本患者32例(观察组),经腹部旋切取出标本患者34例(对照组)。比较两组总手术时间、取标本时间及术后切口愈合情况。【结果】两组患者性别、年龄、体质量指数(BMI)、既往手术史等比较差异均无统计学意义(P〉0.05)。对照组和观察组总手术时间分别为(57.8±10.3)min、(40.3±12.5)min,取标本时间分别为(25.2±3.6)min、(8.7±1.1)min,切口愈合不良率分别为23.5%(8/34)、3.1%(1/32),两组总手术时间、取标本时间及切口愈合不良率比较有统计学意义(Pdo.05);对照组和观察组术后下床活动时间分别为(18士2)h、(19±3)h,胃肠道功能恢复时间分别为(20±3)h、(21±4)h,住院时间分别为(9±1)d、(9±1)d,组间比较均无统计学意义(P〉0.05)。【结论】在腹腔镜巨大子宫全切术中,经阴道“削苹果”取出标本有微创、手术时间短、低风险及愈合好等优点,值得临床推广应用。
[Objective]To explore the clinical evaluation of vaginal "Pare Apple" speeirnen extraction on laparoscopic total hysterectomy for large uterus.[Methods]66 cases suitable for laparoscopic total hysterectomy for large uterus were collected and divided according to their wishes the study group ,32 cases by vaginal "Pare Apple" specimen extraction rand the control group 34 cases by Abdominal peeling specimen extraction. The clinical data such as the total operative time, specimen extraction time and postoperative incision healing was collected and analyzed retrospectively to evaluate this approach.[Results]The differences were not significant between the 2 groups like patients age, body mass index, the previous history of surgery ( P 〈0.05). There were significant differences between the study group and the control group in terms of operative time (57.8±10.3 min VS 40.3±12.5 min), the time of specimen extraction (25.2±3.6 min VS 8.7±1.1 min), and postoperative incision healing [.23.5%(8/34)VS 3.1%(1/32)] (each P 〈0.05). The differences were not significant between the 2 groups in terms of the time to get out of bed (18±2 h VS 40.3±12.5h ), the recovery time of gastrointestinal function(20 ± 3 h VS 21 ±4 h), and hospital stay(9± 1 d VS 9 ±1 d) (each P 〉0.05). [Conclusion]The vaginal "Pare Apple" for specimen extraction is minimally invasive, short operative time, low risk and approved to be a good healing method which is worth further promotion in hospitals.
