右美托咪定对晚期癌痛患者睡眠功能障碍的影响 被引量:5

Effect of Dexmedetomidine on Sleep Disorders in Patients with Advanced Cancer

摘要 [目的]评价右美托咪定(Dex)对晚期癌痛患者睡眠功能障碍的影响。[方法]选择口服硫酸吗啡控释片镇痛的晚期癌痛患者60例,采取随机数字表法分为2组:观察组(D组)和对照组(C组)。D组:静脉缓慢输注负荷剂量Dex 0.5~1.0μg/kg后,以0.2~0.5μg/kg·h持续输注5~7h;C组给予等容量的生理盐水,每天1次,30天为一个疗程。记录治疗期间日均30min内入睡、觉醒〉1次和需要吗啡即释片治疗爆发痛的晚期癌痛患者例数,记录治疗期间晚期癌痛患者日均不良反应发生情况。采用匹茨堡睡眠质量指数(PSQI)进行睡眠质量评估:PSQI总分〉7分为睡眠功能障碍,记录治疗前后PSQI总分〉7分的晚期癌痛患者例数。[结果 ]与C组比较,D组治疗期间日均30min内入睡的患者例数明显增加、觉醒〉1次和需要吗啡即释片治疗爆发痛的晚期癌痛患者例数明显减少(P均〈0.05)。D组治疗期间心动过缓明显增加(χ2=4.320,P=0.038),恶心呕吐明显减少(χ2=5.454,P=0.020),呼吸抑制、低血压的发生率差异无统计学意义。治疗一个疗程后,组内PSQI评分情况:与治疗前比较,D组PSQI总分〉7分的晚期癌痛患者例数明显减少(χ2=4.593,P=0.032);C组PSQI总分〉7分的晚期癌痛患者明显增加(χ2=4.444,P=0.035)。[结论 ]Dex对晚期癌痛患者睡眠功能障碍有预防和治疗作用,能提高晚期癌痛患者的睡眠质量。
