结直肠癌患者术后营养支持效果的对照研究 被引量:10

摘要 比较早期肠内肠外营养支持与肠外营养支持对结直肠癌患者术后的临床疗效。将112例行开腹结直肠癌手术的患者随机分为早期肠内肠外营养支持组(EEN+PN组,n=57)与全肠外营养支持组(TPN组,n=55),分别于术前及术后第5天监测患者的体重指数、血清白蛋白、前白蛋白、转铁蛋白、血脂、血淋巴细胞计数、血C反应蛋白、血清皮质醇、血白介素-2变化情况,比较两组患者术后胃肠道功能恢复时间、营养支持费用及住院天数。术前两组患者的营养相关指标、免疫及应激指标差异均无统计学意义(P>0.05)。术后第5天,EEN+PN组血清前白蛋白、白蛋白、淋巴细胞计数水平均显著高于TPN组(P<0.05);血C反应蛋白、血清皮质醇、血白介素-2、胆固醇、低密度脂蛋白水平均显著低于TPN组(P<0.05)。EEN+PN组术后胃肠道功能恢复时间、住院时间短于TPN组(P<0.05),营养支持费用低于TPN组(P<0.05)。结直肠癌患者术后应用肠内肠外营养支持可以更好的改善机体营养状态,提高机体免疫功能并降低应激反应程度,促进胃肠道功能恢复,降低营养支持费用并缩短住院时间。
