摘要
目的 :探讨三重带袢纽扣钢板固定联合喙锁韧带重建对肩锁关节脱位的治疗作用。方法 :选取我院2012年1月-2014年1月期间骨外科住院部肩锁关节脱位患者89例,采用随机数字表法将患者分为两组,观察组患者采用三重带袢纽扣钢板固定联合关节镜下喙锁韧带重建术,对照组采用锁骨钩钢板固定术,比较两组的临床疗效。结果 :手术前,两组患者Constant各维度(疼痛程度、日常生活活动、外展90°肌力和主动活动范围)评分、喙锁间隙和肩锁间隙比较差异无统计学意义(P>0.05),手术后1年,全部患者Constant各维度评分均明显高于手术前(P<0.05),且观察组明显高于对照组(P<0.05);全部患者喙锁间隙和肩锁间隙均明显小于手术前(P<0.05),且观察组明显小于对照组(P<0.05);观察组术中出血量、手术和住院时间均明显少于对照组(P<0.05)。结论 :三重带袢纽扣钢板固定联合关节镜下喙锁韧带重建对肩锁关节脱位的治疗作用显著,值得临床推广应用。
Objective: To investigate the therapeutic effect of triple endobutton plates fixation combined with arthroscopic coracoclavicular ligaments reconstruction on acromioclavicular joint dislocation. Methods: Eighty-nine cases of patients with acromioclavicular joint dislocation hospitalized from January, 2012 to January, 2014 in our department were selected and randomly divided into an observation group treated by triple endobutton plates fixation combined with arthroscopic coracoclavicular ligaments reconstruction and a control group treated by clavicular hook plate fixation. The clinical efficacy was compared between two groups. Results: There were no significant differences in Constant(pain, daily living activity, 90° abduction strength and motion active range) dimensions scores, coracoclavicular gap and acromioclavicular gap between two groups before the operation(P0.05). All Constant dimensions scores were significantly higher one year after the surgery than preoperation(P0.05) and in the observation group than in the control group(P0.05). The gaps of all patients were significantly smaller postoperation than preoperation(P0.05) and in the observation group than in the control group(P0.05). The volume of intraoperative bleeding and the times for operation and hospitalization were significantly less in the observation group than in the control group(P0.05). Conclusion: The therapeutic effect of this therapy in observation group is significant and it is worthy of clinical application.
