摘要
目的总结疏风解毒胶囊对慢性阻塞性肺疾病急性加重期(acute exacerbations of chronic obstructive pul-monary disease,AECOPD)的治疗作用及对急性期患者营养学指标的影响。方法随机将130例AECOPD患者分为对照组与治疗组,对照组予抗感染等治疗,治疗组在对照组基础上加用疏风解毒胶囊。观察2组治疗后Pa CO2指标变化,治疗前后肿瘤坏死因子α(TNFα)、前白蛋白、转铁蛋白等指标变化,以及观察期间患者药物相关不良反应。结果治疗10 d后,治疗组总有效率为75.4%,对照组为58.5%,其差异比较有统计学意义(P<0.05);治疗组患者动脉血气Pa O2高于对照组[(76.8±7.9)mm Hg vs.(72.1±7.3)mm Hg]、Pa CO2低于对照组[(45.7±5.9)mm Hg vs.(47.6±6.3)mm Hg],其差异比较有统计学意义(P<0.05);治疗组TNFα低于对照组[(29.9±4.7)pg/ml vs.(36.3±6.3)pg/ml],前白蛋白、转铁蛋白高于对照[(227.4±30.5)mg/L vs.(193.3±27.8)mg/L,(2.33±0.33)g/L vs.(1.97±0.28)g/L],其差异均有统计学意义(P<0.05)。结论疏风解毒胶囊联合抗感染等治疗能提高AECOPD的治疗效果,降低患者TNFα水平,改善患者肺功能及相关营养学指标。
Objective To observe the clinic effect of Shufeng Jiedu capsules(SFJDC) on AECOPD, and to evaluate its affect on the status of nutrition. Methods 130 patients were randomly divided into the control group and the treatment group. Patients of the control group received basic treatment. Patients of the treatment group were treated with basic treatment and SFJDC. The patients in the two groups were treated for 10 d. The curative effect, level of TNFα, albumin, transferrin, and the changes of arterial blood gas(PaO2, PaCO2) were assessed. The drug related adverse reactions in patients were evaluated. Results After 10 d, the total effective rate was 75.4% in the treatment group, 58.5%(P 〈 0.05) in the control group. PaO2 in the treatment group was higher than the control group[76.8±7.9 vs. 72.1±7.3, P 〈 0.05], PaCO2 was lower than the control group[45.7±5.9 vs. 47.6±6.3, P 〈 0.05]; TNFα was lower than the control group[29.9±4.7 vs. 36.3± 6.3, P 〈 0.05], and the prealbumin and transferrin levels were higher than the control group[227.4±30.5, 2.33±0.33 vs. 193.3±27.8, 1.97±0.28, P 〈 0.05]. Conclusion SFJDC could facilitate recovery of patients with AECOPD, improve lung function, reduce the patients' level of TNFα,and improve nutrition associated indicators.
