脑出血钻孔引流术手术时机与术后再出血的相关性研究 被引量:1

摘要 目的:探讨高血压脑出血钻孔引流术的手术时机与术后再出血之间的相关性。方法回顾性分析我院2008年2月至2014年11月收治的80例高血压性脑出血患者的临床资料,根据手术时间将患者分为超早期组(发病6小时内手术,42例)、早期组(发病6~24小时手术,22例)和延期组(发病24小时后手术,16例),患者均采用微创钻孔引流术治疗。分析不同手术时机与术后发生再出血率的相关性。结果术后共有18例患者发生再出血,其中超早期组15例、早期组2例、延期组1例。超早期组再出血发生率明显高于早期组(P<0.05),亦明显高于延期组(P<0.05)。三组再出血发生率相比差异具有统计学意义(P<0.05)。结论手术时机与术后再出血具有一定的相关性,超早期手术后再出血的发生率最高,其次为早期手术和延期手术,选择合适的手术时机,可降低术后再出血的发生率,降低脑出血的致残率和病死率。 Objective To study the correlation between surgical timing and postoperative bleeding after cerebral hemorrhage drilling drainage. Methods Clinical data of 80 patients with hypertensive cerebral hemorrhage treated with minimally invasive drilling drainage in our hospital from February 2008 to November 2014 were retrospectively analyzed. All patients were divided into super early group (received surgery within 6 h after onset, 42 cases), early group (received surgery after 6~24 h of the onset, 22 cases) and delayed group (received surgery after 24 h of the onset, 16 cases). The correlation between surgical timing and postoperative bleeding were analyzed. Results After surgery, rebleeding occurred in 18 patients, with 15 cases in super early group, 2 cases in early group and 1 case in delayed group. The incidence of rebleeding in super early group was higher than that of early group and delayed group (P〈0.05), and the incidence of rebleeding between three groups also had statistical difference (P〈0.05). Conclusions The operation time has a certain correlation with postoperative bleeding, the incidence of rebleeding of super early operation is highest, followed by early surgery and delayed surgery. Appropriate operation time can reduce the incidence of postoperative rebleeding, reduce the disability rate and mortality of cerebral hemorrhage.
