摘要
骨骼肌的能量代谢是目前糖尿病领域的研究热点,在骨骼肌中,线粒体功能缺陷是胰岛素抵抗发生的重要机制。中医的肝脾肾在骨骼肌能量代谢方面发挥着重要的作用,肝藏血,为罢极之本;脾统血,为仓廪之官;为肾藏精,为作强之官,精血互生,为运动提供能量;肝主筋,脾主肉,肾主骨,筋骨肌肉相连,是运动的组织基础,肝脾肾协调配合,共同改善疲劳、增强肌肉的能量代谢、减轻并逆转胰岛素抵抗。文章分别从机体运动——骨骼肌能量代谢——线粒体的生理病理影响入手,探讨肝脾肾在糖尿病的发生发展与治疗过程中的意义,为进一步研究糖尿病的发病机制,病机演化及预防治疗提供理论基础。
Mitochondrial function defect is an important mechanism of the insulin resistance in skeletal muscle, in which energy metabolism is diabetes research hot spot now. In traditional Chinese medicine, the liver, the spleen and the kidney play an important role in skeletal muscle energy metabolism. Liver can store blood and spleen can control blood while kidney store Jing(vital essence) in Chinese medicine. Jing and blood can generate each other, which is the source of the energy. What's more, in traditional Chinese medicine, the liver can be in charge of the tendons as well as the spleen in charge of the muscles and the kidney in charge of the bone. The combination of the tendons and the muscles and the bone is the foundation of organization of motion. These three organs coordinate to improve energy metabolism in skeletal muscle, relieve fatigue, and reverse insulin resistance. From the views of motion, energy metabolism in skeletal muscles and the physiological and pathological effects of mitochondria, we indicate that the liver, spleen and kidney play an indispensable role in the development and treatment of diabetes. It could provide a theoretical foundation for research the pathogenesis and the prevention and treatment in diabetes.
