癫(癎)患者睡眠结构变化及其影响因素分析 被引量:1

The analysis of sleep structure and its influence factors in epilepsy

摘要 目的 观察不同类型癫(癎)患者的睡眠结构特征,分析影响癫(癎)患者睡眠质量的相关因素.方法 利用多导睡眠监测技术对46例癫(癎)患者和13例非癫(癎)对照者进行整夜睡眠监测.结果 全面性癫(癎)患者的N2睡眠期的比例明显高于对照组[(71.33 ±11.91)% vs (55.54±6.94)%,P <0.05],而N3睡眠期的比例[(9.75 ±10.70)% vs (18.31±4.64)%,P<0.05]和快速动眼期(REM)比例[(8.58±6.32)%vs(17.77±4.66)%,P<0.05]显著低于对照组,无法分类的发作REM睡眠潜伏期明显低于对照组及其他发作类型(P<0.05),其他发作类型之间睡眠结构差异无统计学意义(P>0.05).年龄>35岁的癫(癎)患者容易出现睡眠效率下降(P=0.014),发病年龄>35岁的癫(癎)患者易出现入睡困难(P=0.038).结论 全面性癫(癎)患者存在明显的睡眠结构改变,年龄>35岁的癫(癎)患者易出现睡眠效率下降,发病年龄>35岁的癫(癎)患者容易出现入睡困难. Objective To investigate sleep structure characteristics in different types of epilepsy, and to analyze the factors affecting sleep quality in epilepsy.Methods Forty six epilepsies and 13 controls were assessed with polysomnography.Results N2 [(71.33 ± 11.91) % vs (55.54 ± 6.94) % , P 〈 0.05]in generalized epilepsy was significantly higher than controls.N3[(9.75 ± 10.70)% vs (18.31 ± 4.64)%,P 〈0.05] and rapid eye movement stage(REM)[(8.58±6.32)% vs (17.77±4.66)%, P 〈 0.05] in generalized epilepsy were significantly lower than controls.The sleep latency of REM in unelassification of attack was significantly lower than other groups(P 〈 0.05).Age older than 35 years old in epilepsy was easy to have decreased sleep efficiency(P =0.014) , and the onset age after 35 years old in epilepsy regularly had difficulty in falling asleep (P =0.038).Conclusions Generalized epilepsy has obviously sleep structural abnormalities;Age older than 35 years old in epilepsy is easy to have decreased sleep efficiency, and the onset age after 35 years old in epilepsy regularly has difficulty in falling asleep.
