摘要
目的：探讨穴位注射联合中药热奄包治疗腰椎间盘突出症的临床效果，并分析其相应的护理措施。方法选择2012年1月~2014年12月本院收治的80例腰椎间盘突出症患者，将其随机分为对照组和观察组，各40例。对照组采用单纯穴位注射治疗及相应的常规护理措施，观察组在对照组基础上采用中药热奄包及护理干预。两组患者均随访1个月，比较两组患者干预后疼痛视觉模拟评分（VAS）、直腿抬高角度评分、下肢麻木缓解时间、腰部疼痛缓解时间、临床治疗效果及治疗过程中发生的护理相关并发症。结果干预后，观察组的疼痛VAS和直腿抬高角度评分低于对照组（P〈0.05），下肢麻木及腰部疼痛缓解时间均显著短于对照组（P〈0.05），治疗无效率低于对照组（P〈0.05）。两组均发生局部感染1例，观察组联合采用中药热奄包治疗，未见局部红肿烫伤及水泡形成者。结论穴位注射结合中药热奄包治疗腰椎间盘突出症的效果理想，结合有效的护理干预，能较快缓解患者的疼痛，改善其腰部症状，提高治疗效果，且治疗不良反应少。
Objective To explore the clinical effect of point injection combining with hot package with Chinese herbs in the treatment of protrusion of lumbar intervertebral disc,and analyze the corresponding nursing measures. Methods 80 patients with protrusion of lumbar intervertebral disc admitted into our hospital from January 2012 to December 2014 were selected and divided into control group and observation group in random,40 cases in each group.In control group,only point injection and conventional nursing measures was applied,while in observation group,hot package with Chinese herbs and nursing intervention was provided on the basis of the control group.Two groups were followed up for 1 month.Score of pain in visual analogue scale （VAS） after interventions,score of straight leg-raising angle,relieving time of numbness of lower limbs,alleviation time of lumbago,clinical therapeutic effect and relative complications by nursing during therapeutic process in two groups were compared. Results After intervention,score of VAS and straight leg-raising angle in the observation group were lower than those of control group （P〈0.05）.Lower limb numbness and pain relief time in the observation group were shorter than those of control group （P〈0.05）.The non-effective rate after therapy in the observation group was lower than that of control group （P〈0.05）.Local infection occurred in one case in both groups,of which hot package with Chinese herbs was used and there was no swelling,scald or blister developed. Conclusion In the treatment of protrusion of lumbar intervertebral disc,point injection combining with hot package with Chinese herbs can obtain an optimal effect.Together with effective nursing intervention, it can quickly alleviate patient’s pain,better waist symptoms,and improve therapeutic effect with few adverse reactions during treatment.
