持续肠内营养对脑卒中吞咽困难患者治疗效果及生活质量的影响 被引量:10

Effect of continuous enteral nutrition on the treatment effect and quality of life in stroke patients with dysphagia

摘要 目的:探讨持续肠内营养对脑卒中吞咽困难患者治疗效果及生活质量的影响。方法:将80例脑卒中吞咽困难患者随机分为观察组和对照组各40例,观察组采取持续肠内营养支持治疗,对照组采取传统多次肠内营养支持治疗。对两组患者鼻饲2周内发生肠内营养并发症情况,患者鼻饲前、鼻饲1周时、鼻饲2周时的营养指标,鼻饲前、鼻饲2周时的生活质量评分,以及鼻饲2周后吞咽障碍治疗有效率、住院死亡率、住院天数进行观察比较。结果:观察组肠内营养并发症发生率和呕吐、反流、误吸发生率均显著低于对照组(P<0.05);观察组鼻饲1周时和鼻饲2周时血红蛋白、前白蛋白、白蛋白、淋巴细胞计数等营养指标均显著高于对照组(P<0.05);鼻饲2周时,两组患者生活质量评分均高于鼻饲前(P<0.05),且观察组评分高于对照组(P<0.05);观察组治疗有效率高于对照组(P<0.05),住院死亡率和住院时间均显著低于对照组(P<0.05)。结论:与传统肠内营养支持治疗相比,持续肠内营养支持治疗可提高脑卒中合并吞咽困难患者的营养水平和治疗有效率、降低其肠内营养并发症发生率和住院死亡率、缩短其住院时间、提高患者生活质量,有利于促进患者的康复和改善患者的短期预后。 Objective:To investigate the effect of continuous enteral nutrition on the treatment effect and quality of life in patients with dysphagia. Methods:80 stroke patients with dysphagia were randomly, divided into the observation group and the control group (40 cases in each group). The continuous enteral nutrition supportive therapy was used in the observation group and the traditional multiple enteral nutrition supportive therapy was given to the patients in the control group. The incidence of complications ,the score of life quality, the effective rate of dysphagia treatment, mortality of inpatient and hospitalization days were observed and compared between the two groups before enteral nutrition supportive therapy and in one week and 2 weeks after enteral nutrition supportive therapy. Results :The incidence of enteral nutrition related complications and the occurrence rate of vomiting,regurgitation and aspiration were significantly lower in the observation group than the control group ( P 〈 0.05 ) ; the nutritional indexes of the patients such as hemoglobin, prealbumin, albumin and lymphocyte count were significantly higher in the observation group than the eontrol group after one week and 2 weeks of nasal feeding ( P 〈 0.05 ) ; the scores of quality of life of the patients were higher after 2 weeks of nasal feeding than those before nasal feeding in the two groups ( P 〈 0. 05 ) ; the scores of quality of life of the patients were higher in the observation group than the control group after 2 weeks of nasal feeding ( P 〈 0. 05 ) ; the effective rate was higher in the observation group than the control group ( P 〈 0.05 ) ; the hospitalization time and mortality of inpatient were significantly lower in the observation group than the control group ( P 〈 0.05 ). Conclusion : Compared with the traditional enteral nutrition supportive method,the continuous enteral nutrition supportive therapy can improve the nutritional level and treatment efficiency, reduce the incidence of complications and mortality, shorten the length of hospital stay and improve the quality of life of the patients.
