摘要
目的:探讨人乳头瘤病毒(HPV)-DNA分型联合宫颈脱落细胞类型检测在宫颈癌早期诊断中的价值。方法:350例宫颈病变患者,应用PCR反向点杂交法和液基薄层细胞检测(TCT)检测其宫颈分泌物HPVDNA类型及宫颈脱落细胞类型,以子宫颈组织的病理学检查结果为依据,比较单纯HPV-DNA分型检测、单纯TCT检测、HPV-DNA联合TCT检测在子宫颈癌早期诊断中的阳性率、灵敏度、特异度及准确性。结果:HPVDNA分型、TCT检测及阴道镜组织病理活检阳性率分别为32.3%、29.7%及20.6%,HPV-DNA分型和TCT检测均出现假阳性;以病理学检查结果为判断依据,HPV-DNA联合TCT检测对子宫颈癌诊断的阳性率及特异度,与单纯HPV-DNA或TCT检测比较,差异无统计学意义(P>0.05);但HPV-DNA联合TCT检测对子宫颈癌诊断的灵敏度高于单纯HPV-DNA或TCT检测(P<0.05),准确度虽升高,但仅与TCT检测差异有统计学意义(P<0.05)。结论:HPV-DNA联合TCT检测可显著提高宫颈癌早期诊断的灵敏度和准确性,对诊断特异度无影响。
Objective: To observe the value of human papillomavirus( HPV) DNA typing combined with cervical cell type detection in early diagnosis of cervical cancer. Methods: A total of 350 patients with cervical lesions were selected as the research objects. PCR reverse dot blot hybridization and thinprep cytologic test( TCT) were applied to detect HPV-DNA type and cervical cell type in cervical secretion. The pathological biopsy results obtained from vagina of the patients were used as the gold standard. The positive rates of PCR DNA typing,TCT and pathological biopsy were compared. Sensitivity,specificity and accuracy of PCR DNA typing combined with TCT were observed. Results: The positive rates of PCR DNA typing,TCT and pathological biopsy were 32. 3%,29. 7% and 20. 6% respectively. Both PCR DNA typing and TCT showed false positive. There were no statistically significant differences in positive rate and specificity between PCR DNA typing or TCT and PCR DNA typing combined with TCT( P〉0. 05). Sensitivity of PCR DNA typing combined with TCT was significantly higher than that of single HPV-DNA or TCT( P〈0. 05),and accuracy increased,the difference was statistically significant compared with TCT( P〈0. 05). Conclusions: The combined application of HPV-DNA typing and TCT can significantly improve diagnostic sensitivity and accuracy of early cervical cancer,and has no effect on the diagnostic specificity.
