基于红外显微镜的衰减全反射法在胆囊结石成分定性分析中的应用价值

摘要 目的 探讨基于红外显微镜的衰减全反射(ATR)法在胆囊结石成分定性分析中的应用价值。方法 选取2014年12月~2015年5月在我院行保胆取石术的60例患者的胆囊结石作为研究对象,分别采用基于红外显微镜的ATR法和传统压片透射法对结石样本进行分析,比较结石成分检出情况、结石分类情况和结果一致性。结果传统压片透射法可检出的结石成分红外显微镜均可检出,红外显微镜可检出的结石成分种类更多,如磷酸盐、脂肪酸钙、蛋白等“罕见”结石成分检出率提高,混合结石检出率也相应提高。两种检出方法判定结石类型结果一致的有42例,占70%;部分相同的18例,占30%,无完全不同的结果。结论 与传统压片透射法相比,基于红外显微镜的ATR法的准确度和敏感度更高,可作为胆囊结石分析的一种有效方法进行推广应用。 Objective To investigate the application value of attenuated total reflection (ATR) method based on infrared microscope in gallbladder stone qualitative analysis. Methods 60 patients with gallbladder stones in our hospital from December 2014 to May 2015 were selected as research object.ATR method and traditional transmission method were both applied to analyze the stone samples.The results of detection of each component,stone classification and result consistency were compared respectively between the two methods. Results Gallbladder stone components that trans- mission method detected could all be detected by ATR method.More components could be detected by ATR method than transmission method.Detection rate of "rare" stone components such as phosphate,calcium stearate and protein was raised by ATR method, accordingly the detection rate of mixed stone was also raised.Between the two methods,42 (70%) of both results were totally identical,18 (30%) were partially identical and no inconsistent result was found. Conclusion Compared with traditional transmission method,ATR method based on infrared microscope has higher sen- sitivity and accuracy so that it can be promoted in gallbladder stone qualitative analysis as an effective method.
