摘要
目的了解山东省首次出现的2例人感染H7N9禽流感疫情特点及病毒血凝素基因变异情况,为制定人感染H7N9禽流感预防控制措施提供科学依据。方法从采集自2例感染者的标本中扩增H7N9禽流感病毒血凝素基因(HA)并测序,获得序列与其他H7N9禽流感病毒血凝素基因进行比对分析,并对该起疫情作流行病学调查。结果对2例确诊病例感染H7N9病毒HA基因的序列分析显示,病毒的受体结合位点均为禽源的,且病毒粒子只存在4个氨基酸位点的变异。结论病例1和病例2感染的H7N9病毒为同一株(核苷酸同源性为100%),与环境分离株H7N9禽流感病毒不属于同一来源(核苷酸同源性99.82%)。本起人禽流感疫情的感染源可能暴露于被H7N9禽流感病毒污染的环境,病例1的传播可能是由环境或禽到人的传播。病例2为有限人传人的可能性最大,即病例1传给病例2,而共同暴露于环境感染的几率较小。因此认为山东省首起H7N9禽流感疫情为输入性疫情,存在有限的人传人。
ObjectivesTo investigate and analyze 2cases of human infection with avian influenza H7N9 in order to ascertain mutations in the hemagglutinin(HA)gene of the avian influenza H7N9 virus and to provide a scientific basis for control of human infection with the avian influenza virus. Methods Specimens were collected from patients infected with the avian influenza H7N9 virus and the virus' HA gene was sequenced.The genes were compared and the outbreak was investigated epidemiologically. Results Sequencing of the HA gene of the avian influenza H7N9 virus from 2patients with a confirmed infection indicated that the virus' receptor binding sites had come from the avian form of the virus and that virus particles only had mutations at 4amino acids. Conclusion Both individuals in case 1and case 2were infected with the same strain of the avian influenza H7N9virus(nucleotide similarity:100%),though this strain did not have the same source as avian influenza H7N9 virus isolated from the environment(nucleotide similarity:99.82%).The outbreak of avian influenza in humans may be due to exposure to an environment contaminated with the avian influenza H7N9 virus.In case 1,the virus may have been transmitted from the environment or from birds.In case 2,person-to-person transmission was most likely limited.The individual in case 1transmitted the infection to the individual in case 2;joint exposure to an environmental infection is less likely.The first cases of human infection with avian influenza H7N9 in Shandong Province were imported,and person-to-person transmission of the virus is limited.
