摘要
目的了解不同水碘地区成人碘营养状况和甲状腺相关疾病患病情况。方法采用多阶段随机抽样法,对天津市不同水碘地区506名调查对象进行B超检查、甲状腺生化指标检测和尿碘检测;采集并检测居民户盐样、所在行政村(居委会)水样。结果高碘地区水碘中位数155.7μg/L,非高碘地区水碘中位数8.0μg/L。高碘地区食用无碘食盐,非高碘地区食用加碘食盐,盐碘中位数23.30mg/kg。高碘地区成人尿碘中位数168.10μg/L大于非高碘地区146.20μg/L(P=0.048),均处于适宜水平;非高碘地区TGAb、TPOAb阳性率(22.5%、19.3%)均高于高碘地区(11.8%、9.8%);慢性淋巴细胞性甲状腺炎患病率为高碘地区(5.02%)>非高碘地区(1.03%);甲状腺结节患病率为高碘地区(32.48%)>非高碘地区(15.09%)。结论高碘地区成人甲状腺结节、慢性淋巴细胞性甲状腺炎患病率要高于非高碘地区,应引起重视。
Objective To understand iodine nutritional status and prevalence of thyroid related diseases among adults in areas with different water iodine levels in Tianjin. Methods A multi-stage random sampling method was used to enroll 506 adults from high iodine and non-high iodine areas in Tianjin into the study.B-ultrasound was performed,thyroid biochemical parameters and urinary iodine levels were determined.Salt and water samples were also collected for iodine test. Results The median iodine concentration of water samples from high and non-high iodine areas were 155.7μg/L and 8.0μg/L,the overall median iodine concentration of salt was 23.30 mg/kg.Meanwhile,the median iodine concentration in urine samples from high iodine area was higher than that from non-high iodine area（168.10μg/L vs 146.20μg/L,P=0.048）.In non-high iodine area,the positive rates of TGAb and TPOAb（22.5%,19.3%）were higher than that of high iodine area（11.8%,9.8%）,while the prevalence of chronic lymphocytic thyroiditis and thyroid nodules were both lower（1.03%vs 5.02%,15.09%vs 32.48%）. Conclusions The prevalence of adult thyroid nodules and chronic lymphocytic thyroiditis in high iodine area are higher than that of in non-high iodine area.
