摘要
目的探讨血清神经特异性烯醇化酶(NSE)、髓鞘碱性蛋白(MBP)、S-100蛋白在病毒性脑炎患儿中的变化及临床意义。方法 2012年1月-2013年12月选取本院收治的48例病毒性脑炎患儿为脑炎组。根据患儿入院7 d内病情分为重症组28例及危急重症组20例。另随机选取本院30例健康儿童为对照组。2组入院当天抽取静脉血及脑脊液(CSF)各2 ml,采用免疫荧光法测定MBP水平,酶联免疫法测定NSE、S-100水平。结果脑炎组患儿血清及CSF中NSE、MBP和S-100水平显著高于对照组,差异有统计学意义(P<0.05)。与重症组相比,危急重症组患儿血清及CSF中NSE、MBP和S-100水平显著升高,差异有统计学意义(P<0.05)。脑炎组中预后良好的患儿血清及CSF中NSE、MBP和S-100水平显著高于预后差的患儿,差异有统计学意义(P<0.05)。结论脑脊液和血清中NSE、MBP和S-100水平与病毒性脑炎患儿病情进展及预后具有密切的关系。通过测定三者在脑脊液和血清中含量将有助于对患儿的病情进行评估。
Objective To investigate the changes of serum neuron- specific enolase( NSE), myelin basic protein( MBP), S - 100 protein in children with viral encephalitis and the clinical significance. Methods From January 2012 to December 2013, 48 children with viral encephalitis from our hospital were selected as the encephalitis group. According to the condition of chil- dren within 7 d, 28 cases and 20 cases were respectively selected as the emergency group and the severe group. Another 30 ca- ses of healthy children were randomly selected as the control group from our hospital. At 2 d, blood and cerebrospinal fluid (CSF) were sampled for 2 ml respectively, and MBP levels were measured by immunofluorescence, besides, enzyme -linked immunosorbent assay were used to measure NSE, S - 100 levels. Results NSE, MBP and S - 100 levels in blood and CSF of children with encephalitis were significantly higher than that in the control group, with the differences statistically significant ( P 〈 0.05 ). Compared with the emergency group, NSE, MBP and S - 100 levels in blood and CSF of severe children were sig- nificantly higher, and the differences had statistical significance (P 〈 0.05). NSE, MBP and S - 100 levels in blood and CSF of children in the encephalitis group with good prognosis were significantly higher in children with poor prognosis, and the differ- ence was statistical significance (P〈 0.05). Conclusion The NSE, MBP and S - 100 levels in cerebrospinal fluid and serum have a close relationship with disease progression and prognosis in children with viral encephalitis. Measuring the three levels in the cerebrospinal fluid and serum content will help to assess the condition of the children.
