摘要
目的 探讨体外循环下二尖瓣置换术同时行双极射频消融治疗风湿性二尖瓣病变合并心房颤动的临床疗效.方法 选择我院2012-01 ~2015-01间收治的风湿性二尖瓣病变合并心房颤动患者80例作为研究对象,根据不同治疗方式分为两组,对照组40例单独行瓣膜置换术治疗,观察组40例同时行瓣膜置换术+双极射频消融术治疗,比较两组患者的围术期情况及心脏彩超结果.结果 观察组主动脉阻断时间、体外循环时间均高于对照组(P<0.05);术后6个月时,观察组左心室射血分数(LVEF)、左室短轴缩短率均显著高于对照组(P<0.05);观察组术后即刻、术后6个月时窦性心律转复率分别为80.0%、70.0%,均显著高于对照组(P<0.05).结论 体外循环下二尖瓣置换术同时行双极射频消融术,能明显改善风湿性二尖瓣病变合并心房颤动患者左心室功能,更有利于保持患者心率平稳.
Objective Study on clinical efficacy of treating mitral valve replacement under extracorporeal circulation and bipolar radio- frequency ablation on of rheumatic mitral valve disease and atrial fibrillation. Methods From January 2012 to January 2015, the target hospital treated 80 patients with rheumatic mitral valve disease and atrial fibrillation study, according to different treatments are divided into 2 groups, single valve replacement in the treatment of 40 cases in the control group, 40 patients for observation undergoing valve replacement surgery simultaneously and bipolar radiofrequency ablation, compared between the 2 ventricle groups of patients with perioperative cardiac ultrasound and the results. Results Observation group active pulse blocking time, and body outside cycle time were highercontrol group (P 〈 0.05 ) ; 6 months after operation, observation group LVEF, and left room short axis shortened rate were significantly higher than control group ( P 〈 0.05 ) ; immediately after observation group operation, and 6 months after operation, sinus sex rhythm turned complex rates were respectively for 80. 0% and 70.0% , were significantly higher than control group ( P 〈 0.05 ). Conclusion Mitral valve replacement under extracorporeal circulation and bipolar radiofrequency ablation could improve left ventricular function in patients with rheumatic mitral valve disease and atrial fibrillation, which might keep heart rate steady.
