摘要
目的探讨自发型干燥综合征(SS)模型非肥胖糖尿病(NOD)小鼠颌下腺DWI表现及其病理基础。方法对20只11~15周龄雌性NOD小鼠(实验组)颌下腺行常规MR及DWI扫描(b值为0.800 mm^2/s)及病理检查,并以10只雌性BALB/c小鼠为对照组。结果两组小鼠周龄及体质量差异均无统计学意义(P=0.699、0.623)。病理检查HE染色见实验组小鼠均发病,共78个淋巴细胞浸润灶;天狼猩红染色小鼠颌下腺淋巴细胞浸润灶内可见大片红染的胶原纤维。实验组小鼠颌下腺ADC值[(0.751±0.137)×10^(-3)mm^2/s]低于对照组[(0.867±0.126)×10^(-3)mm^2/s],差异有统计学意义(P<0.05)。实验组小鼠颌下腺淋巴细胞浸润程度与ADC值呈负相关(r=-0.777,P<0.05)。以病理结果为金标准,小鼠颌下腺ADC值0.8125 mm^2/s诊断SS的敏感度为75.0%,特异度为80.0%,曲线下面积为0.745(95%CI0.553~0.937)。结论自发型SS动物模型NOD小鼠的颌下腺ADC值减低,并随着淋巴细胞浸润程度加重而下降。
Objective To explore MR DWI findings of submandibular glands Sjogren's syndrome(SS) in non-obese diabetic(NOD) mouse and its pathological basis.Methods Submandibular glands MR DWI(b values=0,800 mm^2/s) and pathological examinations were performed on 20 female NOD mice in 11—15 weeks old(study group).And totally 10 female BALB/c mice were treated as control group.Results There was no significant difference of age and body mass between the two groups(P =0.699,0.623).HE staining pathology confirmed that all the mice of study group were in the pathogenesis of the disease.A total of 78 lymphocytic infiltration lesions were found.And large patchy of collagen fibers redden in submandibular glands lymphocytic infiltration lesions were observed by Sirius red staining pathology.The ADC values in submandibular glands of study group([0.751 ± 0.137]×10^-3 mm^2/s) were lower than that of control group([0.867 ± 0.126]×10^-3 mm^2/s,P〈0.05).The degree of lymphocytic infiltration in submandibular glands of study group were correlated negatively with the ADC values(r= — 0.777,P〈0.05).Taking the results of pathology as the gloden standard,the optimal cutoff point of ADC value for the diagnosis of SS in mice was 0.8125mm^2/s.The diagnostic sensitivity and specificity was 75.0%and 80.0%,respectively.And the area under the ROC curve was 0.745(95%CI 0.553—0.937).Conclusion The ADC value of submandibular glands in NOD mouse with SS decreases with the aggravation of lymphocytic infiltration.
