摘要
目的通过心内科护士巴林特小组案例分析,解析目前护患互动中护士的真实情感体验,探索借助巴林特小组的培训模式以提高临床护理质量。方法在黄浦区中心医院心内科护士心身技能培训中组织6次巴林特小组活动,共呈现6个临床案例,采用扎根理论的方法对讨论记录进行质性分析。结果案例发生的情境主要集中于护患沟通的冲突;小组讨论的主题包括与患者相关的情感体验、职业定位。结论巴林特小组可以帮助参与者识别和处理护患沟通中复杂的情绪反应,澄清职业角色的要求,提高工作满意度,从而促进职业化护患关系能力建设。
Objective To explore the contexts and triggers of cases presented in nurse Balint groups and to clarify the themes in the group discussions to interpret the negative experiences of nurses in nurse-patient inter- action in cardiovascular medicine to promote high-quality service in clinical nursing. Methods In Shanghai Huangpu District Central Hospital, six nurse Balint sessions were organized as part of psychosomatic medicine skills training and six cases were presented. A grounded theory-based approach with the qualitative content analysis of the field notes was used. Results Three triggers for case narrations were identified, including witnessing injus- tice and role confusion, which originated from three distinct contexts, patient encounters, confusing experiences in teamwork, tension between privacy and profession. Three main discussion themes could be identified. Conclusion It suggests that the feelings related to patients and to one's own role as a nurse have been openly discussed in groups. The discussions often touch on professional identity and professional growth and on how to increase the job satisfaction and to promote the professional ability in building the nurse-patient relationship.
