摘要
目的:分析社区卫生服务机构开展艾滋病防治服务的影响因素相对重要性程度大小,为加强基层艾滋病防治能力建设提供政策和科学依据。方法:采用卫生系统宏观模型构建社区卫生服务机构开展艾滋病防治服务的影响因素层次指标体系,通过专家评分问卷对各层影响因素的相对重要性进行评分,再用层次分析法确定各影响因素的相对重要性大小。结果:形成社区卫生服务机构开展各项艾滋病防治服务的3层影响因素指标体系,其中第三层的20个影响因素为社区卫生服务机构开展艾滋病防治服务的指标性影响因素。层次分析的结果提示,按权重系数大小排序居于前6位的影响因素分别为社区领导重视,政策保障,人力质量,人员待遇,人力数量、经费投入、社区医生认可度,工作考核与管理、业务水平。结论:需要各地卫生行政部门提供可操作的政策措施和保障条件,增加社区层面艾滋病防治经费投入,加强艾滋病防治队伍建设,改善服务环境,加强社区卫生工作人员的艾滋病防治知识、技能和职业暴露预防培训,推动社区组织积极参与艾滋病防治服务,长远保障基层艾滋病防治服务的可持续开展。
Objective： To explore the influencing factors and their weightiness of HIV / AIDS prevention practice in Community Health Service Center（ CHSC） so as to provide policy and scientific evidence for HIV / AIDS prevention capacity construction at community level. Methods： A hierarchical structure index was set up for the influencing factors of HIV / AIDS prevention practice in CHSC by using the Health System Macro-Model. An expert- based questionnaire survey was conducted to assess the importance of influencing factors and the Analytic Hierarchy Process（ AHP） method was employed to determine the weightiness of all factors. Results： A three- level factor framework was set up among which 20 factors in the third level were the index factors for HIV / AIDS prevention practice in CHSC. AHP indicated that the top six factors by weight coefficient size were manpower quality,manpower number,financial input,recognition of service providers,policy support,leadership of CHSCs,service environment,involvement of community organizations,skill level of service providers,training and resource input in CHSCs. Conclusion：It is highly advocated that health administrative department should render policy and resource supportability for HIV / AIDS prevention practice in CHSC. Meanwhile measures such as HIV / AIDS financial input,work team construction,technological support,training about HIV / AIDs prevention skill and occupational exposure prevention and involvement of community organizations should be taken to make the HIV / AIDS prevention services sustainable in CHSCs in the long run.
