摘要
目的 探讨妊娠期亚临床甲状腺功能减退症(SCH)对妊娠结局的影响。方法 回顾性分析2012年7月-2014年12月至我院产科门诊行甲状腺功能筛查的2880例妊娠期妇女中,妊娠合并SCH的共58例,其中伴甲状腺抗过氧化物酶抗体(TPO-Ab)阳性的37例,抗体阴性的21例。将抗体阳性组中愿意接受左旋甲状腺素(L-T4)治疗的20例设为治疗组,不愿意接受L-T4治疗的17例设为非治疗组。同时,将抗体阴性组中的21例作设为观察组,未给予L-T4治疗;随机选取同期在我院产检的正常孕妇80例设为抗体阴性组中的对照组,观察两组孕妇的妊娠结局。结果 治疗组的妊娠不良结局发生率低于非治疗组(P〈0.05),观察组的妊娠不良结局发生率与对照组差异无统计学意义(P〉0.05)。结论 SCH伴TPO-Ab抗体阳性的妊娠妇女不良妊娠结局发生的风险系数明显增高,而L-T4治疗可明显改善其妊娠结局。妊娠妇女早期行甲状腺功能筛查并及时治疗对优生优育非常有利。
Objective To explore the impact of subclinical hypothyroidism (SCH) during pregnancy on pregnancy outcome. Methods 2880 pregnant women given screening of thyroid function in department of obstetrical outpatient outpatient in our hospital from July 2012 to December 2014 were retrospectively analyzed.Among that patients,pregnancy combined with SCH was 58 cases,and there was 37 cases with TPO-Ab positive,antibody negative was 21 cases.20 cases in antibody positive group accepted the sinistral thyroxine(L-T4) treatment were selected as treatment group,unwilling accepted L-T4 was selected as non treatment group.At the same time,21 cases in antibody negative group were selected as observation group and not given L-T4 treatment.80 normal pregnancy that examined?in our hospital at the same term were selected as control group in antibody negative group.Pregnancy outcome was observed in pregnant women of two groups. Results The incidence rate of bad pregnancy outcome in treatment group was lower than that of non treatment group (P〈0.05).There was no statistical difference of the incidence rate of bad pregnancy outcome between observation group and control group (P〉0.05), Conclusion The risk coefficient of pregnancy bad outcome occurred increased in SCH with TPO-Ab positive pregnancy,while the treatment with L-T4 can improve the pregnancy outcome.It is helpful to eugenics that pregnancy women given the thyroid function screening in the early stage and treatment timely.
