黄荧光辅助引导技术在颅内恶性肿瘤手术中的初步应用 被引量:4

摘要 目的 探讨黄荧光辅助技术在颅内恶性肿瘤手术中的作用.方法 回顾性分析2014年3月至2014年7月首都医科大学附属北京天坛医院神经外科术前诊断为颅内恶性肿瘤的21例患者的临床资料.21例患者术中均使用装有YELLOW 560 nm滤光片的PENTERO 900荧光显微镜切除肿瘤,根据术中肿瘤的荧光染色情况判断肿瘤边界,结合超声技术进行肿瘤切除.结果 21例患者术中无对比剂相关不良事件发生,术中荧光显影19例,2例未见肿瘤黄荧光染色或弱荧光染色.术后病理诊断:胶质母细胞瘤13例,间变性星形细胞瘤4例,间变性少枝胶质细胞瘤2例,胶质肉瘤1例,转移瘤合并间变性星形细胞瘤1例.结论 对于颅内恶性肿瘤,黄荧光辅助技术有助于辨别肿瘤边界,提高手术的安全性和有效性. Objective To investigate the role of yellow fluorescein-guided technique in the operation of intracranial malignant tumors.Methods From March 2014 to July 2014,the clinical data of 21 patients with intracranial malignant tumor diagnosed before procedure at the Department of Neurosurgery,Beijing Tiantan Hospital,Capital Medical University were analyzed retrospectively.The tumors of 21 patients were resected using the PENTERO 900 fluorescence microscope equipped with YELLOW 560 nm filter during procedure.According to intraoperative fluorescence staining of the tumors,the tumor borders were identified,and the tumors were removed with ultrasound technique.Results No contrast agent-related adverse events occurred in 21 patients during procedure.Nineteen patients had fluorescent development during procedure,no yellow fluorescent staining or weak fluorescent staining of tumors in 2 cases was observed.Postoperative pathological diagnosis:13 patients had glioblastoma,4 had anaplastic astrocytoma,2 had anaplastic oligodendroglioma,1 had glial sarcoma and 1 had metastases complicated with anaplastic astrocytoma.Conclusion For intracranial malignant tumors,yellow fluorescence-guided technique is contribute to identifying tumor borders and improves the safety and effectiveness of the operation.
