摘要
目的 分析血清25羟维生素D[25(OH)D]与颈动脉粥样硬化的相关性。方法 选取2014年-2015年南京鼓楼医院体检者100例,男性76例,女性24例,年龄39-81岁。将患者分为4组,25(OH)D〉30 ng/ml为正常组(9例),20 ng/ml〈25(OH)D≤30 ng/ml为轻度缺少组(38例),15 ng/ml〈25(OH)D≤20 ng/ml为缺少组(35例),25(OH)D≤15 ng/ml为缺乏组(18例)。酶联免疫吸附试验双抗体夹心法检测血清25(OH)D,应用荷兰PHILIPS公司四维彩超IU-Elite测量双侧颈总动脉内膜中层厚度,测量三个心动周期。结果 正常组比较,轻度缺少组、缺少组和缺乏组,颈动脉内膜中层厚度均增加,数值为[(0.58±0.15)mm vs.(0.75±0.22)mm]、[(0.58±0.15)mm vs.(0.92±0.24)mm]、[(0.58±0.15)mm vs.(1.13±0.30)mm],差异有统计学意义(P均〈0.05)。25(OH)D为颈动脉粥样硬化的独立保护因素(OR=0.947,95%CI:0.907-0.990),年龄(OR=1.096,95%CI:1.009-1.191)和尿酸(OR=1.112,95%CI:1.007-1.201)为颈动脉粥样硬化的独立危险因素。Pearson相关分析,颈动脉内膜中层厚度与25(OH)D呈负相关(r=-0.49,P〈0.05)。结论 血清25(OH)D水平与颈动脉内膜中层厚度呈负相关,是颈动脉粥样硬化的独立保护因素。
Objective To analyze the correlation between serum 25 hydroxy vitamin D [25(OH)D] and carotid atherosclerosis. Methods The patients(n=100, male 76, female 24 and aged from 39 to 81) were chosen from 2014 to 2015, and then divided into group A(25(OH)D30 ng/m L, n=9), group B(20 ng/m L25(OH)D≤30 ng/m L, n=38), group C(15 ng/m L25(OH)D≤20 ng/m L, n=35) and group D(25(OH)D≤15 ng/m L, n=18). The level of serum 25(OH)D was detected by using double antibody sandwich ELISA, and carotid artery intima-media thickness(CAIMT) was detected for 3 cardiac cycles. Results Compared with group A, CA-IMT increased in group B, group C and group D [(0.58±0.15) mm vs.(0.75±0.22) mm,(0.58±0.15) mm vs.(0.92±0.24) mm,(0.58±0.15) mm vs.(1.13±0.30) mm, all P〈0.05]. 25(OH)D was the independent protective factor of carotid atherosclerosis(OR=0.947, 95%CI: 0.907-0.990), and age(OR=1.096, 95%CI: 1.009-1.191) and uric acid(OR=1.112, 95%CI: 1.007-1.201) were the independent risk factors for carotid atherosclerosis. Pearson correlation analysis showed that CA-IMT was negative related to 25(OH)D(r=-0.49, P〈0.05). Conclusion The level of serum 25(OH)D is negatively related to CA-IMT, and is the independent protective factor of carotid atherosclerosis.
