摘要
目的研究单独采用高强度聚焦超声(HIFU)治疗子宫肌瘤的疗效。方法 2012年2月-2014年2月共145例子宫肌瘤患者接受HIFU治疗,共治疗174个肌瘤。治疗前及治疗后次日、6个月、12个月行MRI检查及随访,分析治疗后肌瘤缩小率、不良反应、临床症状改善情况。结果 145例患者的174个肌瘤均成功完成HIFU消融治疗。1有效性:治疗前和治疗后次日行MRI平扫加增强检查及治疗后6、12个月行MRI平扫检查,显示肌瘤体积治疗前为4.61-419.70 cm^3,中位体积56.82 cm^3,治疗后次日为3.02-578.21 cm^3,中位体积56.74 cm^3,消融体积为2.42-578.21 cm^3,中位消融体积47.84 cm^3,体积消融率为24%-100%,治疗前后肌瘤体积差异无统计学意义(P〉0.05);肌瘤体积治疗后6个月为0-264.50 cm^3,中位体积22.49 cm^3,12个月为0-346.02 cm^3,中位体积14.81 cm^3,治疗后6、12个月肌瘤体积平均缩小了60.4%和73.9%,治疗前与治疗后6、12个月两个时间点的肌瘤体积差异有统计学意义(P〈0.05),6个月与12个月差异有统计学意义(P〈0.05)。中位子宫肌瘤症状(UFS)及中位生活质量(QOL)评分在消融治疗前分别为21.88分和71.55分,消融治疗后6、12个月与消融治疗前比较,UFS分别降低了57.1%、71.4%,QOL分别升高了15.7%、26.5%,治疗前与治疗后6、12个月两个时间点的UFS及QOL差异有统计学意义(P〈0.05),6个月与12个月差异有统计学意义(P〈0.05),随时间推移UFS逐渐减轻,QOL逐渐提高。2安全性:发生皮肤浅Ⅱ度热损伤2例,下肢放射痛1例,一过性尿潴留1例,并发症发生率2.8%,随访中有6例复发。结论 HIFU治疗子宫肌瘤安全、有效,是对于在传统手术治疗中无法保留子宫或者患者拒绝手术治疗的另一种选择。
Objective To probe into the clinical ei cacy of high-intensity focused ultrasound(HIFU) alone for uterine i broids. Methods Between February 2012 and February 2014, 145 patients with 174 cases of uterine i broids were treated with HIFU. Before and one day, 6 months, 12 months after treatment, MRI was performed during the follow-up. We analyzed i broids narrowing rate, adverse reactions, and clinical symptoms improvement at er treatment. Results Successful completion of HIFU ablation was done for all the i broids. Enhanced MRI examination before and one day at er treatment, and MRI plain scanning 6 and 12 months at er treatment showed that i broid volume before treatment was 4.61-419.70 cm^3(median of 56.82 cm^3), i broid volume at er treatment was 3.02-578.21 cm^3(median of 56.74 cm^3), ablation volume was 2.42-578.21 cm^3(median of 47.84 cm^3), and volumetric ablation rate was 24%-100%; there was no statistically signii cant dif erence in i broid volume before and at er treatment(P〈0.05); i broid volume 6 months at er treatment was 0-264.50 cm^3(median of 22.49 cm^3) and was 0-346.02 cm^3(median of 14.81 cm^3) 12 months
