超声骨刀联合翻瓣术治疗重度慢性牙周炎的疗效观察 被引量:11

摘要 目的:比较超声骨刀和高速涡轮钻在牙周翻瓣骨手术中的疗效。方法:选择重度慢性牙周炎患者27例共61个患牙,随机分为实验组(n=31):超声骨刀处理;对照组(n=30):高速涡轮钻处理。观察两组治疗前、后临床牙周指标BI、PD、CAL的变化,并比较两组间疗效差异;同时观察两组术后第2、4天术区有无疼痛、肿胀的发生。结果:术前两组BI、PD、CAL牙周指标无显著差异(P>0.05)。术后6周,两组各项牙周临床指标均较术前有明显改善(P<0.05);两组间各项牙周指标比较无显著差异(P>0.05)。术后第2天,实验组发生疼痛和肿胀的例数均少于对照组(P<0.05);术后第4天,两组并发症无显著性差异(P>0.05)。结论:两种手术方法均可有效提高重度慢性牙周炎的临床疗效,但超声骨刀能更好的减少术后并发症。 AIM: To compare the clinical effects of periodontal osseous surgery by using piezosurgery and high- speed turbine handpiece. METHODS: 27 patients with 61 teeth of severe periodontitis were treated by periodontal flap surgery using piezosurgery( group P,n = 31) and high- speed turbine( group H,n = 30) respectively.Bleeding index( BI),periodontal depth( PD) and clinical attachemtn loss( CAL) were observed before and after surgery. The difference of pain and swelling was analyzed statistically on the 2nd and 4th day after operation. RESULTS:Before treatment there was no difference of all indices between 2 groups. 6 weeks after operation the indices improved( P〈0. 05) and there was no difference between the 2 groups( P〉0. 05). Group P had less pain and swelling on the2 nd postoperative day than group H( P〈0. 05),there was no difference on the 4nd postoperative day( P〈0. 05).CONCLUSION: Piezosurgery and high-speed turbine are effective in the flap surgery for the treatment of severe chronic periodontitis. Piezosurgery device is more effective in reducing postoperative complications than high-speed turbine handpiece.
