摘要
[目的]基于病案探析国医大师何任先生治疗胃痛的经验。[方法]通过整理何老本人的著述、何老门人弟子辑录的何老验案医论以及公开发表的期刊杂志,共收集何老论治胃痛案38案,将此38案分别编号后进行分析,总结何老论治胃痛的经验。[结果]胃痛38案中,肝气犯胃20案,何老用自拟脘腹蠲痛汤为主进行治疗。肝胃阴虚9案,用一贯煎为主方进行加减;肝旺脾虚2案,治以四逆散加四君子汤;寒邪客胃轻证1案,用良附丸,重证2案,用乌头赤石脂丸;脾胃虚弱1案,用黄芪建中汤;胃痛兼见肺热、遗精各1案,分别治以千金苇茎汤和金锁固精丸;阴虚胃痛1案,治以清润和降之膏方。[结论]何老辨治胃痛重视脾胃分治,其证治心法以八字为纲,尤重虚实和重视肝胃关系。
[Objective] To analyze experience of diagnosis and treatment of stomachache of Mr. He Ren, the Chinese medicine master, based on medical records.[Methods]Collect medical records of stomachache which treated by Mr. He Ren from printed book and articles. There are 38 cases all together, we number these medical records and analyze them one by one, so that we could summarize the experience of diagnosis and treatment of stomachache of Mr.He Ren. [Results]There is 20 cases involving liver Qi attacking the stomach, which treated by Wanfu Juantong Tang, and 9 cases involving liver and stomach yin deficiency, which treated by Yiguan Jian, and 2 cases involving hyperactivity of liver and spleen deficiency, which treated by Sinisan combined with Sijunzi Tang, and 1 case involving mild cold evil guest stomach, which treated by Liangfu Wan, and 2 cases involving severe cold evil attacking stomach, which treated by Wutouchishizhi Wan, 1 case of weakness of the spleen and the stomach, treated with Huangqi Jianzhong Tang, 1 case of stomachache accompanied with lung heat, which treated by Qianjing Weijing Tang, and 1 case of stomachache accompanied with spermatorrhea, which treated by Jingsuo Gujing Wan, and 1 case involving stomachache due to Yin deficiency, which treated by soft extract. [Conclusion]The experience of diagnosis and treatment of stomachache of Mr.He Ren includes distinguishing the treatment of spleen and stomach, emphasizing distinguish, ment of deficiency and excess,and valuing the relationship between liver and stomach.
