摘要
目的 探讨分娩巨大儿的非糖尿病孕妇血清视黄醇结合蛋白4(Retinol-binding protein 4,RBP4)和胰岛素抵抗指数(Insulin Resistance,IR)与非糖尿病性巨大儿发生的关联。方法 测定583例非糖尿病孕妇妊娠12周、24周、37周的血清RBP4水平,同时测定空腹血糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINs),应用HOMA稳态模型(FPG×FINs/22.5)评估胰岛素抵抗指数(HOMA-IR);随访研究对象至足月分娩,以出生体重≥4 000g者为巨大儿,出生体重2 500-4 000g者为正常体重儿;以分娩巨大儿的42例非糖尿病孕妇作为研究组,从年龄、孕产次可比的、分娩正常体重儿的非糖尿病孕妇中随机选取50例作为对照组,回顾性分析两组孕妇在妊娠12周、24周、37周时的血清RBP4水平及HOMA-IR的差异。结果 巨大儿的发生率为7.2%(42/583);研究组和对照组血清RBP4水平及HOMA-IR均随着孕周增加而升高,妊娠37周高于妊娠24周(P〈0.05),妊娠24周与妊娠12周相比差异无显著性(P〉0.05);研究组妊娠24周及37周的RBP4及HOMA-IR均高于对照组(P均〈0.05),组间妊娠12周水平无显著性差异(P〈0.05)。结论 血清RBP4参与胰岛素抵抗及非糖尿病性巨大儿的发生。
Objective To investigate the correlation of Retinol- binding Protein 4( RBP4),insulin resistance( IR) and Non- diabetic large infant. Methods Calculating the serum value of RBP4,fasting plasma glucose( FPG) and fasting insulin( FINs) of 583 cases of Non- diabetic maternal in pregnancy of 12 weeks,24 weeks and 37 weeks; Using the Homeostasis model assessment( HOMA) steady state model to evaluate insulin resistance( HOMA- IR); Follow up all pregnant women to full term delivery and consider 42 cases of pregnant women who gave birth to large for date infants as study group and chose 50 cases of pregnant women who gave birth to normal for date infants as control group,then retrospective analyze difference of RBP4 and HOMA- IR of the two groups. Results Of the 583 non- diabetic maternal,42 cases( 7. 2%,42 /583) gave birth to large infants; The serum value of RBP4 and HOMA- IR of all pregnant women raised with the increase of gestational weeks,the values of RBP4 and HOMA- IR of 37- week pregnancy were higher than which of 24- week pregnancy( P〈0. 05),but there was no statistical difference between the value of 24- week pregnancy and which of 12- week pregnancy( P〉0. 05); The values of RBP4 and HOMA- IR of 24,37- week pregnancy of study group were higher than which of control group( P〈0. 05),but there was no significant difference between the value of 12- week pregnancy of the two groups( P〉0. 05). Conclusion RBP4 participate the development of IR,and RBP4 and IR might play an important role in the development of non- diabetic large for date infant.
