QRS时限和左室射血分数及左心室舒张末期容积与心脏性猝死关系 被引量:5

摘要 目的 探讨QRS时限﹑左室射血分数(left ventricular ejection fraction,LVEF)﹑左心室舒张末期容积(left ventricular end-diastolic volume,LVEDV)与心力衰竭患者发生心脏性猝死(sudden cardiac death,SCD)的关系。方法对263例心力衰竭患者进行随访,依据转归分为SCD组和存活组,比较2组入院时QRS波时限、LVEF、LVEDV,非条件logistic回归分析SCD发生的危险因素。结果随访24~36(24.5±3.4)个月,23例发生SCD;SCD组QRS波[(119.94±14.31)ms]较存活组延长[(90.30±12.21)ms],LVEF[(35.10±6.22)%]较存活组降低[(56.23±6.14)%],LVEDV[(59.31±4.82)%]较存活组[(53.15±2.90)%]增大,差异均有统计学意义(P〉0.05);logistic回归分析显示,QRS波延长(RR=4.021,P=0.023)、LVEF降低(RR=7.424,P=0.004)、LVEDV增大(RR=4.023,P=0.024)为SCD发生的独立危险因素。结论 心力衰竭患者发生SCD与QRS时限延长、LVEF降低、LVEDV增加有关。
