摘要
目的调查住院糖尿病患者肺部感染的病原学与耐药性,总结干预措施,为该病的早期治疗提供参考依据。方法选取2014年1-12月医院收治的118例糖尿病发生肺部感染患者为研究对象,采集患者的痰标本,分析病原菌分布构成及其耐药性。结果共分离出病原菌120株,其中革兰阴性菌74株占61.7%,革兰阳性菌32株占26.7%,真菌14株占11.6%;鲍氏不动杆菌对妥布霉素耐药率为0,肺炎克雷伯菌对阿米卡星、亚胺培南耐药率为0,铜绿假单胞菌对呋喃妥因、妥布霉素耐药率为0,大肠埃希菌对呋喃妥因、美罗培南、亚胺培南耐药率为0,金黄色葡萄球菌及表皮葡萄球菌对呋喃妥因耐药率均为0,真菌中白色假丝酵母菌无耐药菌检出。结论肺炎克雷伯菌是糖尿病患者发生肺部感染最常见的病原菌,病原菌的总体耐药性不容乐观,对患者应采取综合干预措施,合理应用抗菌药物。
OBJECTIVE To investigate the etiology of pulmonary infections in the hospitalized patients with diabetic mellitus,analyze the drug resistance,and put forward the intervention measures so as to provide guidance for early treatment of the disease.METHODS A total of 118 diabetes patients with pulmonary infections who were treated in the hospital from Jan 2014 to Dec 2014 were recruited as the study objects,then the sputum specimens were collected from the enrolled patients,and the distribution and drug resistance of the pathogens were observed.RESULTS Totally 120 strains of pathogens were isolated,including 74(61.7%)strains of gram-negative bacteria,32(26.7%)strains of gram-positive bacteria,and 14(11.6%)strains of fungi.The drug resistance rate of the Acinetobacter baumannii to tobramycin was 0,the drug resistance rates of the Klebsiella pneumoniae to amikacin and imipenem were 0,the drug resistance rates of the Pseudomonas aeruginosa to nitrofurantoin and tobramycin were 0,the drug resistance rates of the Escherichia coli to nitrofurantoin,meropenem,and imipenem were 0,and the drug resistance rates of the Staphylococcus aureus and Staphylococcus epidermidis to nitrofurantoin were 0.Among the fungi,no strain of drug-resistant Candida albicans was found.CONCLUSION The K.pneumoniae is the most common species of pathogen isolated from the diabetes patients with pulmonary infections.The status of overall drug resistance of the pathogens is not optimistic.It is necessary to take comprehensive intervention measures and reasonably use antibiotics.
