乳腺肌上皮癌3例临床病理特征分析 被引量:6

摘要 目的 探讨乳腺肌上皮癌的临床表现、组织形态学改变和免疫表型特征。方法 采用HE及免疫组化Leica BOND-MAX全自动染色仪Bond Polymer Refine Detection法,对3例乳腺肌上皮癌进行染色,并复习相关文献。结果 乳腺肌上皮癌的癌组织多由梭形细胞组成,部分病例可见上皮样细胞、浆样细胞和透明细胞,呈实性片状、肺泡样、条索状排列;核分裂象易见;无坏死;可发生腋窝淋巴结转移。免疫表型:3例均表达CK、EMA、CD10、p40、actin、Calponin、p63;2例表达CK5/6、CK14、SMA;1例表达S-100蛋白、D2-40、CAM5.2;CD117、CD34、ER、PR、C-erbB-2/HER-2和desmin均不表达;Ki-67增殖指数为10%-20%。结论 肌上皮癌细胞形态多样,结合免疫组化组合标记可做出明确诊断,治疗方式以乳腺癌根治术为主。 Purpose To study the clinical manifestation, histological change and immunohistochemical phenotype of breast myoepithelial carcinoma. Methods Routine HE and immunohistochemical method (Leica BOND-MAX automatic, Bond Polymer Refine Detection) were stained for 3 cases of breast myoepithelial carcinoma, and the relevant literatures were also reviewed. Results The tumor were predominantly composed of spindle cells. In some cases, epithelioid cells, plasma-like cells and clear cells coule seen. These cells were arranged in solid, alveolar or cords. Mitosis was seen easily, and necrosis had not been observed. In some cases, axillary lymph node metastasis had been occurred. The result of immunohistochemical staining showed that CK, EMA, CD10, p40, actin, Calponin and p63 were expressed in all cases. CK5/6, CK14 and SMA were expressed in 2 cases. S-100, D2-40 and CAM5.2 were ex- pressed in 1 cases. CD117, CD34, ER, PR, C-erbB-2/HER-2 and desmin were negative in all cases. The proliferation index of Ki- 67 was 10% - 20%. Conclusion The histological changes of myoepithelial carcinoma cells are diverse, and should be careful to make a correct diagnosis with the results of immunohistochemical staining. The treatment of this disease is mainly based on surgery, and traditional breast cancer radical operation is the best choice.
