The Pathological Grades for Distal Cholangiocarcinoma:Evaluation by 3.0 T MR Diffusion Weighted Imaging

摘要 目的探讨3.0 T MR扩散加权成像(DWI)序列评估远端胆管癌病理分化程度的价值。方法 20例远端胆管癌按病理分化程度分组,分析不同分化程度组胆管癌的DWI图像;观察病灶的信号强度并测量表观扩散系数(ADC)值。结果 20例远端胆管癌在DWI上主要呈稍高信号和高信号(95%)。不同分化程度组间胆管癌的ADC值分别为(1.684±0.166)×10-3mm2/s,(1.443±0.135)×10-3mm2/s,(1.266±0.040)×10-3mm2/s,(1.190±0.031)×10-3mm2/s.胆管癌的ADC值随分化程度的降低逐渐下降(rs=0.928,P=0.000)。各组间ADC值的差异有统计学意义(P<0.05)。结论 3.0 T MR DWI序列能够一定程度地判断远端胆管癌的病理分化程度。 Objective To evaluate the value of diffusion weighted imaging (DWI) on 3.0 T MRI in determining the pathological grades of distal eholangiocarcinoma. Methods Twenty patients who were clinically and histologically diag- nosed with distal cholangiocarcinoma underwent abdominal MRI ( including DWI ). Signal intensity and ADC values for DWI were noted. Results Slightly hyperintensity or hyperintensity were found in 19 patients on DWI (95% , 19/20). The ADC values for different pathologic grades were (1. 684±0. 166)×10^-3mm^2/s, (1. 443 ± 0. 135 )×10^-3 mm^2/s, ( 1.266 ±0. 040)×10^-3mm^2/s and ( 1. 190±0. 031 )×10^-3mm^2/s in patients with cholangioearcinoma. The ADC values were correlated with the pathological grades ( r = 0. 928, P = 0. 000). There Were statistical differences for ADC values a- mong different pathological groups ( P 〈 0.05 ). Conclusion The pathological grades for distal cholangioearcinoma can be evaluated by 3.0 T MR DWI sequence.
