摘要
目的探讨保乳术联合术后放疗在年轻女性早期乳腺癌患者中的应用价值。方法 120例年轻女性早期乳腺癌患者,年龄均≤40岁,试验组48例接受患侧乳腺区段切除术加腋窝淋巴结清扫术,术后均辅助放疗;对照组72例接受患侧乳腺癌改良根治术。评价术后1年美容效果,观察两组患者术后5年死亡率和局部复发率、远处转移率。结果试验组与对照组术后5年死亡率、局部复发率和远处转移率分别为2.08%、4.17%、6.25%和2.78%、4.17%、6.94%,两组比较,差异无统计学意义(P>0.05),而试验组乳房外观美容效果优于对照组(P<0.01)。结论保乳术联合术后放疗治疗年轻女性早期乳腺癌患者,与改良根治术相比,不增加死亡率、局部复发率及远处转移率,术后能够获得良好的美容效果。
Objective To investigate the application value of breast-conserving surgery combined with postoperative radiotherapy for young women with early-stage breast cancer. Methods A total of 120 young women (age ≤ 40 years) with early-stage breast cancer were divided into two groups. Forty-eight patients in the experimental group underwent partial mastectomy of the affected side combined with axillary-lymph-node dissection, and received adjuvant radiotherapy postoperatively. Seventy-two patients in the control group received modified radical mastectomy. The postoperative cosmetic efficacy was evaluated one year after surgery. The 5-year mortalities as well as the incidences of local recurrence and distant metastasis were compared between two groups. Results The 5-year mortalities, incidences of local recurrence and distant metastasis of experimental group and control group were 2.08% vs. 2.78% ,4.17% vs. 4.17% and 6.25% vs. 6.94%, respectively, and no statistically significant difference was found between two groups ( P 〉 0.05 ). The postoperative cosmetic efficacy in the experimental group was superior to that in the control group (P 〈 0.01 ). Conclusion For young women with early-stage breast cancer, breast-conserving surgery combined with postoperative radiotherapy might not increase the mortality as well as the incidences of local recurrence and distant metastasis ,and it can obtain better postoperative cosmetic efficacy.
