摘要
目的目前无公认的方法可快速建立种植体周围炎(种周炎)骨缺损的动物模型。为种周炎骨缺损后骨再生的深入研究提供一种快速建模途径。方法选择6只比格犬,每只比格犬即刻植入6枚种植体。待种植体-骨结合3个月后,采用随机数字表法抽取3只比格犬作为实验组,翻瓣局部去骨+丝线结扎+高糖饮食综合作用;其他3只作为对照组,行常规Ⅱ期手术。1个月后分别通过种植体牙周指标检查、种植体周围龈沟液(periimplantsulcularfluid,PISF)测定、X线片、直观测量和组织切片,观察种植体周围骨缺损程度[缺损深度(DD值)、缺损宽度(DW值)、牙槽骨水平(BL值)]并进行对比研究。结果实验组PISF含量较对照组显著升高[(1.95±0.28)μL vs(1.02±0.14)μL,P<0.01]。实验组可见植体颈部牙槽嵴高度下降且植体间牙槽间隔消失,对照组则未见明显骨吸收,2组垂直骨丧失量差异有统计学意义[(3.08±0.84)mm vs(0.32±0.08)mm,P<0.05]。实验组口内植体表面均被大量菌斑和软垢覆盖,牙龈不同程度充血红肿,局部翻瓣暴露植体后均可见螺纹暴露,出现不同程度的骨缺损,DD值、DW值、BL值较对照组差异均有统计学意义[(3.18±1.36)mm vs(0.26±0.08)mm,(2.38±0.73)mm vs(0.21±0.04)mm,(1.70±0.79)mm vs(0.15±0.05)mm,P<0.05]。组织切片显示实验组种植体颈部破骨细胞增生活跃。结论局部去骨+丝线结扎+高糖饮食联合法可快速建立种周炎骨缺损模型,适合在动物科研中推广应用。
Objective To further provide a fast modeling approach for the study of bone regeneration due to the bone defect of the peri - implantitis Methods Six beagles were randomized to two groups, and were extracted respectively four premolars ( a total of eight roots). Then, the DIO implants were transplanted immediately into six suitable extraction sockets. Three months after osseoin- tegration, the combined therapy of local boning, silk thread ligation, and high glucose diet were conducted in the experimental group. For the controls, the traditional second stage surgery was performed. After one month, the implant bone defects were observed, and compared with the controls by performing the periodontal and X-ray examination, measuring periimplantsulcular fluid, and observing both the visual and tissue sections change. Results After treating for one month, both the periodontal index and PISF of the experimental group improved significantly when compared with the controls (P 〈 O. 05 ) ; We observed an obvious bone defect in the ex- perimental group, also, visual change was detected ( DD = 3.18 mm, DW = 2. 38 mm, BL = 1. 70 mm). Further, there is a significant difference when compared with the control group( DD =0.26 mm,DW= O. 21 mm, BL = 0.15 mm) (P 〈 0.05 ) ; The tissue sections revealed that the proliferation status of osteoclasts was active at the top of implant in the experimental group. Conclusion We identified that the combined method of local boning, silk thread ligation and high glucose diet can help quickly construct the peri-implantitis bone defect animal model, thus proving a novel and useful method of studying the bone generation derived from a bone defect of the peri-implantitis.
