耐碳青霉烯类肠杆菌科细菌耐药基因研究进展 被引量:13

摘要 随着碳青霉烯类抗生素在临床上广泛使用,肠杆菌科细菌的耐药性问题日益严重。产碳青霉烯酶是耐碳青霉烯类肠杆菌科细菌(CRE)主要的耐药机制,编码这类酶的耐药基因包括A、B、D三类。A类酶主要以肺炎克雷伯菌碳青霉烯酶(KPC)为主;B类酶主要是在印度泛滥的新德里金属β内酰胺酶(NDM),可出现在肺炎克雷伯菌和大肠埃希菌中;D类酶主要以肺炎克雷伯菌和大肠杆菌产苯唑西林酶-48(OXA-48)为主。大多数产KPC型酶菌株将造成院内感染,而产NDM、OXA-48酶菌株可造成医院和社区感染。虽然大部分CRE的耐药分子类型影响范围较为局限,但是其播散范围呈扩大趋势,且新近很多突变类型出现,值得广泛关注。
