摘要
[目的]观察血腑逐瘀汤联合西药治疗冠心病心绞痛疗效。[方法]使用随机平行对照方法,将100例住院及门诊患者按掷骰子法简单随机分为两组。对照组50例降压、降糖;硝酸异山梨酯,10mg/次,2次/d;阿司匹林75mg/次,1次/d;硝苯地平,10mg/次,1次/d;心功能不全加地高辛0.25mg/次,1次/d。治疗组50例血腑逐瘀汤(赤芍16g,牛膝20g,桃仁25g,当归20g,枳壳16g,红花20g,川芎14g,桔梗14g,甘草3g;脾胃虚弱加白术、党参、山药各12g;心烦、失眠加柏子仁、酸枣仁各12g;剧烈疼痛加蒲黄16g,延胡索20g,五灵脂16g;胸闷异常加薤白20g,瓜萎22g;严重心悸、自汗加龙骨^(先煎)15g,生牡蛎^(先煎)16g;血瘀闭经加益母草12g,香附15g),1剂/d,水煎300mL,早晚口服,150mL/次;西药治疗同对照组。连续治疗28d为1疗程。观测临床症状、心绞痛发作频率、不良反应。连续治疗2疗程,判定疗效。[结果]临床疗效:治疗组显效20例,有效24例,无效6例,总有效率88.00%。对照组显效8例,有效27例,无效15例,总有效率70.00%。治疗组疗效优于对照组(P<0.05)。心电图疗效:治疗组显效21例,有效25例,无效4例,总有效率92.00%。对照组显效6例,有效26例,无效18例,总有效率64.00%。治疗组疗效优于对照组(P<0.01)。[结论]血腑逐瘀汤联合西药治疗冠心病心绞痛疗效满意,无严重不良反应,值得推广。
[Objective]To observe the blood Xuefuzhuyutang Combined Western Medicine Angina.[Methods]Randomized parallel controlled method,100 inpatients and outpatients by dice method is simple randomized into two groups.Control group of 50 patients with hypertension,diabetes,blood pressure hypoglycemic therapy line.Isosorbide dinitrate,10mg/times,2 times/d;aspirin,75mg/times,1 times/d;nifedipine,10mg/times,1 times/d;ventricular dysfunction plus digoxin,0.25mg/times,1 times/d.Treatment group of 50 patients Xuefuzhuyu soup（Chishao 16 g,Niuxi 20 g,Taoren 25 g,Danggui 20 g,Zhike 16 g,Honghua 20 g,Chuanxiong 14 g,Jiegeng 14 g,Gancao 3g;spleen and stomach plus Baizhu,Dangshen,Shanyao each 12g;irritability,insomnia plus Bozi,Suanzaoren each 12g;severe pain plus Puhuang 16 g,Yanhusuo 20 g,Wulingzhi 16g;chest abnormal add Xiebai 20 g,Gualou 22g;severe heart palpitations,spontaneous plus Longgu 15 g,Shengmuli 16g;blood stasis amenorrhea plus Yimucao 12 g,Xiangfu 15g）,1/d,decoction 300 m L,sooner or later,oral,150 m L/times.Western medicine with the control group.28 d for a course of continuous treatment.Observation of clinical symptoms,angina frequency,adverse reactions.2 courses of continuous treatment,to determine efficacy.[Results]Clinical efficacy： treatment group 20 cases,effective 24 cases,6 cases,the total efficiency of 88.00%.A control group that were 8 cases,effective 27 cases,15 cases ineffective,the total efficiency 70.00%.Treatment group than the control group（P〈0.05）.ECG efficacy： treatment group 21 cases,effective 25 cases,4 cases,the total efficiency of 92.00%.A control group that were 6 cases,effective 26 cases,ineffective in 18 cases,the total efficiency of 64.00%.Treatment group than the control group（P〈0.01）.[Conclusion]Blood Xuefuzhuyutang Combined with Western treatment of angina pectoris results were satisfactory,no serious adverse reactions,it is worth promoting.
