摘要
目前认为胃肠道是全身炎症反应综合征(SIRS)的始动器官,是发生发展多器官功能障碍综合征(MODS)的枢纽器官,是脓毒症的发源地,急性胃肠功能障碍一旦发生在危重症患者中,则往往提示病情加重或预后不良。在临床危重症病情发展过程中,在其他脏器功能尚未出现损害表现前,胃肠道功能会首先出现急性受损,但却极易受临床医师忽视,将会影响其对基础疾病的及时处理和对严重度和预后的预先判断。因此,正确观察和认识危重症急性胃肠功能障碍的发生形式和演变过程,及时加以积极处理干预,并以此对原发疾病的预后作出准确和早期的判断已成为提高当代危重症诊疗成功率的一个极其重要的方面。
At present, the gastrointestinal tract is considered to be the initiating organ of systemic inflammatory response syndrome (SIRS), the hub organ for occurrence and development of multiple organ dysfunction syndrome (MODS), and the birthplace of sepsis. Once critically ill patient develops acute gastrointestinal dysfunctionin, it suggests exacerbations or poor prognosis. In the development of critical illness, acute damage will first appear in gastrointestinal function, before damage on other organs, but this symptom is extremely easy to be ignored by the clinician, which can affect the timely treatment of basic diseases and pre judgment of severity and prognosis. Therefore, correct observation and understanding of the occurrence and development of acute gastrointestinal dysfunction in critically ill patients, timely treatment, and accurate and early judgment of primary diseases prognosis plays important role in improving the success rate of critical illness diagnosis and treatment in modern time.
