摘要
目的通过疾病累计评分量表(CIRS)和欧洲五维健康量表(EQ-5D),评价老年心房颤动患者共病状态程度与生活质量,探讨共病状态和生活质量的相关性。方法选择住院确诊的134例老年心房颤动患者,分为卧床组73例和非卧床组61例,分别应用CIRS和EQ-5D对患者进行评分。结果老年心房颤动患者CIRS总分为(14.62±4.33)分,严重指数为1.03±0.31,共病指数为4.52±1.37,EQ-5D总分为(10.89±3.02)分。卧床组CIRS评分与EQ-5D评分与非卧床组比较有显著性差异[(15.29±4.67)分vs(13.80±8.78)分,P=0.04;(12.19±2.45)分vs(9.33±2.90)分,P=0.00],CIRS总分与共病指数(r=0.998,P=0.000)、严重指数(r=0.942,P=0.000)均呈显著正相关,严重指数与血肌酐水平(r=0.350,P=0.000)、血尿酸水平(r=0.276,P=0.002)呈显著相关。结论老年心房颤动患者共病状态与生活质量相关,CIRS与EQ-5D的应用,可以客观评价共病状态的严重程度与生活质量。
Objective To study the association between comorbidity and quality of life in elderly atrial fibrillation（AF）patients by assessing their comorbidity and life quality according to the cumulative illness rating scale（CIRS）and Europe health-related quality of questionnaire 5-dimension（EQ-5D）.These patients were divided into two groups,73 patients in group of patient in bed and 61 patients in group of patient not in bed.Methods The comorbidity and quality of life in 134 elderly AF patients admitted to our hospital were scored according to the CIRS and EQ-5D.Results The total CIRS score,average severity index,average comorbidity index and average EQ-5Dscore were 14.62±4.33,1.03±0.31,4.52±1.37 and 10.89±3.02,respectively.The scores of CIRS and EQ-5Din group of patient in bed and group of patient not in bed were significantly different in elderly AF patients（15.29±4.67 vs 13.80±8.78,P=0.04;12.19±2.45 vs9.33±2.90,P=0.00）.The score of CIRS was significantly positive correlated with the index of comorbidity（r=0.998,P=0.000）and the index of severity（r=0.942,P=0.000）.The average severity index showed the severity of disease better than the average comorbidity index,and was closely associated with the serum creatinine and uric acid levels（r=0.350,P=0.000;r=0.276,P=0.002）.Conclusion The comorbidity is associated with the quality of life in elderly AF patients whose comorbidity and quality of lfe can be assessed according to the CIRS and EQ-5D.
