摘要
目的探究临床上对于骨科患者的损伤控制（Department of orthopedics damage control,DOC）在具有不稳定的骨盆骨折同时伴有四肢多发性的骨折（Unstable pelvic fractures combined with multiple fractures of the extremities,UPFMFE）患者中应用的临床效果。方法随机选取2013年6月-2015年6月收治的20例患有不稳定的骨盆骨折同时伴有四肢多发性的骨折的患者。根据DOC原则对患者进行一期的抗休克治疗,患者的骨盆骨折在临床上均使用骨盆带进行固定以用来止血,在ICU病房对患者进行复苏治疗以后,再对患者实施确定性的内部固定手术。首先,先实施四肢骨折的内部固定手术,然后再进行骨盆骨折的内部固定手术。手术后,对患者随访6-18个月,复查患者的X射线。结果 20例患者最终均被救治成功,对骨折的复位比较满意。其中4例尿道有损伤的患者经治疗后重建,没有尿道狭窄的存在。4例患者因为手术前同时伴有坐骨神经的牵拉伤,在手术后具有短时间的肢体障碍,剩余的16例患者在手术后均可以生活自理.手术后对患者随访6-18个月,对其进行X射线复查,结果显示,均为骨性愈合,没有畸形愈合的存在,也没有远期的并发症存在。结论临床上合理的使用DOC的原则用于治疗UPFMFE患者,提供了一种崭新的治疗理念,保证了患者救治的连续性以及整体效果,其疗效确切,安全。
Objective To investigate the clinical for orthopaedic patients with damage control（ Department of orthopedics damage control,DOC） with unstable pelvic fractures associated with limb multiple fractures（ unstable pelvic fractures combined with multiple fractures of the extremities,UPFMFE） in patients with clinical effect. Methods a random selection of 20 patients with unstable pelvic fractures and multiple fractures of the extremities were treated in our hospital from June 2013 to June. According to the principle of doc to patients for a period of anti shock treatment,patients with pelvic fracture in the clinical use of pelvis belt fixed to stop bleeding,in the ICU on patients of resuscitation after,for patients with implementation of internal fixation of deterministic. First of all,the first implementation of the internal fixation of the fracture of the limbs,and then the internal fixation of pelvic fractures. After surgery,the patients were followed up for 6 ~ 18 months,and the patients were followed up for X rays. Results 20 cases were treated successfully,and the reduction of fracture was satisfactory. 4 cases of patients with urethral injury after treatment,no urethral stricture. 4 cases of patients because before surgery and associated with sciatic nerve traction injury. After the operation with short time of physical disabilities,the remaining 16 patients after surgery can self- care. Postoperative patients were followed up 6to 18 months. The is review of X- ray. The results showed that,were bone healing. No malunion exist,there is no long- term complications exist. Conclusions the clinical reasonable use doc principles for the treatment of patients with UPFMFE,provides a new idea for the treatment,to ensure the treatment of patients with continuity and the overall effect,its efficacy and safety.
