摘要
目的了解本地区临床感染嗜水气单胞菌的分布特点及耐药情况。方法对该院2009~2013年感染性患者标本中分离的嗜水气单胞菌的分布和耐药性进行回顾性分析。结果 5年间共分离到嗜水气单胞菌189株,其中肠道内感染菌株132株,肠道外感染的57株。肠道外感染的菌株多来源于肝胆疾病(20例)、血液系统肿瘤(10例)等患者的血液(26.3%)、胆汁(21.1%)等标本。嗜水气单胞菌的感染多发于夏秋季,50岁以上的中老年人。肠道内与肠道外感染的嗜水气单胞菌的耐药率存在差别,但对氨苄西林、氨苄西林/舒巴坦、头孢唑啉、阿莫西林-克拉维酸、头孢西丁均有较高的耐药率(〉51.1%),肠道外感染嗜水气单胞菌仅对左氧氟沙星、亚胺培南、阿米卡星、美洛培南较敏感(耐药率〈10%)。结论该地区嗜水气单胞菌的感染高发于夏秋季、中老年人,以肠道内感染常见,但其对抗菌药物的敏感性较好;肠道外感染多见于肝胆疾病、肿瘤等患者,耐药率较高且在不断上升。临床医师应根据感染部位区别对待,参考药敏选药并继续加强监测。
Objective Objective To investigate the clinical characteristics and antimicrobial resistance of Aeromonas hydrophila species. Methods A restrospective analysis for antibiotic resistance and clinical distribution of Aeromonas hydrophila species was applied during 2009 to 2013. Results A total of 189 strains were isolated during the 5 years,of which 132 strains were from intestinal infected patients and 57 from parenteral infected patients. Parenteral infected strains were mainly sourced from blood( 26. 3%),bile( 21. 1%) and other specimens of hepatobiliary disease( 20 cases),blood system tumors( 10 cases) patients. Aeromonas hydrophila infections mainly occurred in summer and autumn among the elderly people over 50 years. There were significant differences in drug resistance rates between the intestinal and parenteral infected Aeromonas hydrophila,but to ampicillin,ampicillin / sulbactam,cefazolin and amoxicillin- clavulanic acid,cefoxitin,they both had relatively high resistance rates( 〉51. 1%). However,parenteral infected Aeromonas hydrophila were only sensitive for levofloxacin,imipenem,amikacin,meropenem( resistance rate 〈10%). Conclusions The Aeromonas hydrophila infection mainly occurs in summer and autumn among the elderly people,and the intestinal infections is common,but general sensitive to antimicrobial drugs; parenteral infections are common in the liver and gallbladder disease,cancer and other immunocompromised patients,drug resistance rates are high and rising; clinicians should be cautious to drug choice according to the site of infection,the antimicrobial susceptibility results and continue to strengthen monitoring research.
