无痛超声输卵管显影术与常规超声输卵管显影术检查输卵管通畅性的对比研究 被引量:3

Accuracy of examining fallopian tube patency with transvaginal salpingosonography under painless anesthesia or waking states

摘要 目的探讨患者在麻醉无痛和常规清醒不同状态下行超声输卵管显影术检查输卵管通畅性的准确性。方法回顾性分析2014年3月至2015年4月在我院辅助生殖中心行超声输卵管显影术、并于检查后行宫-腹腔镜检查术的81例患者资料。其中42例患者在常规状态下(对照组)、39例患者在无痛状态下(观察组)行超声输卵管显影术,共157条输卵管。分析两种检查方法与宫-腹腔镜检查的符合率、敏感度和特异度。结果对照组超声输卵管显影术输卵管通畅性指标:符合率71.6%(58/81),诊断输卵管阻塞的敏感度为74.2%(23/31),输卵管通畅的特异度为75.0%(30/40);研究组输卵管显影术输卵管通畅性指标:符合率85.5%(65/76),诊断输卵管阻塞的敏感度为89.7%(26/29),输卵管通畅的特异度为91.9%(34/37)。研究组诊断输卵管通畅性更准确(χ2=4.48,P<0.05)。结论无痛超声输卵管显影术在检查诊断输卵管通畅性方面有显著性优势,能够有效排除输卵管痉挛所导致的假性梗阻,诊断输卵管通畅性更加准确可靠。 Objective: To investigate the accuracy of examining fallopian tube patency with transvaginal salpingosonography(TSSG)under the different condition,i.e.painless in anesthetized or conventional waking states.Methods: The data of 81 infertile patients performed transvaginal salpingosonography and hysteroscopy-laparoscopy for fallopian tube patency from March 2014 to April 2015 were retrospectively analyzed.Among them,42 patients were performed transvaginal salpingosonography under painless in anesthetized,and 39 patients were under waking states.The diagnostic coincidence rate,sensitivity and specificity were compared between the two methods.Results: The diagnostic coincidence rate of the conventional TSSG for fallopian tube patency was71.6%(58/81),with a sensitivity 74.2%(23/31)and specificity 75.0%(30/40).The diagnostic rate of the painless anesthesia TSSG for fallopian tube patency was 85.5%(65/76),with a sensitivity of 89.7%(26/29)and a specificity of 91.9%(34/37).The diagnostic coincidence rate of two methods for fallopian tube patency was significantly different(χ2=4.48,P〈0.05).Conclusions: The painless anesthesia TSSG had obvious advantage in the diagnosis of the fallopian tube patency with more accuracy.It can effectively eliminate pseudo-obstruction caused by tubal spasm.
