摘要
目的观察右美托嘧定对鼻内镜手术围手术期血流动力学及眼压(IOP)的影响。方法选择我院耳鼻喉科择期行鼻内镜手术患者30例,ASAⅠ~Ⅱ级,将患者随机分入右美托咪定组(D组)和对照组(N组),每组15例。所有患者均采用气管插管全身麻醉。右美托咪定组于气管插管前15 min泵注1μg/kg右美托咪定,之后以0.5μg·kg^(-1)·h^(-1)持续经静脉输液泵注射。N组泵入等量生理盐水。记录T_0(麻醉前),T_1(诱导后)、T_2(插管即刻),T_3(填塞前5 min)、T_4(填塞后5min)、T_5(填塞后10 min)共5个时间点MAP、HR、IOP。结果在T_0时间点两组患者HR、MAP、IOP无显著性差异(P<0.05)。与T_0相比,T_1时两组HR、MAP、IOP均下降,D组较N组下降更为明显(P<0.05);T_2时N组HR、MAP、IOP均显著升高,D组无明显变化(P<0.05);T_4、T_5时N组IOP显著升高,D组无明显变化(P<0.05)。结论右美托嘧啶可以有效降低鼻内镜手术插管反应并预防IOP的升高。
Objective To investigate the influences of Dexmedetomidine on intraocular pressure and hemodynamics during endoscopic sinus surgery.Methods 30 cases underwent endoscopic sinus surgery were randomly divided into control group(n = 15,group N) and experiment group(n = 15 group D).Group D received Dexmedetomidine 1 μg/kg infusion for 15 minutes before intubation,and then received Dexmedetomidine 0.5 μg·kg-1·h-1 target controlled infusion.While group N received the same amount of saline.HR、MAP and IOP were recorded before anesthesia(T0),after intubation(T1),the moment of endotracheal intubation(T2),5 min before nasal cavity filling(T3),at 5 min(T4) and at 10 min(T5) after nasal cavity filling.Results HR、MAP and IOP of group D at T1 were significantly lower than those at TO and those of group N(P 〈 0.05).HR、MAP and IOP of group N at T2 were significantly higher than those of group N(P〈0.05).IOP of group N at T4-5 were significantly higher than those of group N(P 〈 0.05).Conclusion Dexmedetomidine can inhibit fluctuations of hemodynamics and can stable intraocular pressure during endoscopic sinus surgery.
