多普勒超声、磁共振对比增强血管造影在肾动脉狭窄患者中的诊断价值比较 被引量:5

摘要 目的:比较多普勒超声(Doppler ultrasonography,DUSG)、磁共振对比增强血管造影(Contrast-enhanced magnetic resonance angiography,CE-MRA)以及DUSG与CE-MRA的联合使用对肾动脉狭窄(Renal artery stenosis,RAS)的诊断价值。方法:以选择性肾动脉造影(Selective renal arteriography,SRA)为诊断RAS的金标准,将156例患者分为三组:第一组54例患者仅使用DUSG进行诊断,第二组48例患者仅使用CE-MRA进行诊断,第三组42例患者使用DUSG与CE-MRA联合诊断,观察并比较DUSG、CE-MRA这两种影像学检查方法分别使用和联合使用对RAS的诊断效能。结果:两种影像学检查方法分别使用和联合使用对RAS的诊断效能差异有统计学意义(P<0.05)。结论:DUSG是筛查RAS的首选影像学检查方法,DUSG、CE-MRA联合使用对RAS的诊断准确性最高。 Objective: To compare diagnosis value of Doppler uhrasonography ( DUSG ) and contrast- enhanced magnetic resonance angiography ( CE-MRA ), as well as combined use, in renal artery stenosis ( RAS ) . Method: Selective renal arteriography ( SRA ) was used as the gold standard for diagnosis of RAS. 156 patients were divided into 3 groups. In the first group, 54 patients were diagnosed only by DUSG. In the second group, 48 patients were diagnosed only by CE-MRA. In the third group, 42 patients were diagnosed by DUSG combined with CE-MRA. Diagnosis value between the two imaging methods were compared when they were used separately and together. Result: There were statistically significant differences in the diagnosis result of RAS when using the two imaging methods separately and together. Conclusion: DUSG is the first choice of imaging examination method for the screening of RAS, while combined use of DUSG and CE-MRA has the highest level of diagnostic accuracy.
