19例脊柱结核的CT与MRI影像诊断价值探讨 被引量:13

摘要 目的:探讨脊柱结核的CT与MRI影像学表现及应用价值。材料与方法:回顾分析19例确诊为脊柱结核患者的CT与MR影像学表现,对CT及MRI影像学表现进行对比分析。结果:脊柱结核的CT影像主要表现为椎体及其附件的骨质破坏、椎管受累以及椎旁脓肿形成等;脊柱结核MRI影像表现为椎体及附件的骨质破坏、椎间盘信号的改变、椎旁脓肿形成、韧带下播散以及硬膜囊和脊髓受累等。在显示早期病变方面MRI更为敏感,在确定病变范围方面MRI较CT也更具有优势,但是对于病变内钙化灶以及死骨的显示方面CT明显清晰。结论:脊柱结核CT与MRI影像学表现多样,均具有较高诊断价值,两者结合更有利于脊柱结核的诊断及治疗效果评价。 Purpose: To explore the imaging performance and the diagnostic value of CT and MRI imaging in diagnosing spinal tuberculosis. Materials and Methods: The CT and MRI imaging data of 19 cases with confirmed spinal tuberculosis were retrospectively analyzed and compared to CT imaging data and MRI data. Results: The CT imaging features of spinal tuberculosis included vertebral body and accessory bone destruction, vertebral canal involvement and paravertebral abscess formation, etc. The MRI imaging features of spinal tuberculosis showed vertebral body and accessory bone destruction, disc signal involvement, paravertebral abscess formation, the spreading along ligament, dural sac and spinal cord involvement. In finding early lesions, MRI was more sensitive, and in determining the extent of the lesion, MRI also had more advantages, but for showing calcification and dead bone, CT is more clearly evident. Conclusion : The CT and MR imaging of spinal tuberculosis had the characteristics change of diversity. They had high diagnostic value. Combining two methods of diagnosis, will helpful for the diagnosis of spinal tuberculosis and to evaluate its therapeutic effects.
