摘要
目的研究早期动脉血乳酸清除率与重症肺炎预后的相关性。方法选择2014年3月至2015年6月间于我院接受治疗的42例重症肺炎患者作为研究对象。将患者按照不同的标准分为存活组(25例)和死亡组(17例),高血乳酸清除率组(23例)和低血乳酸清除率组(19例)。观察比较患者的初始血乳酸水平、治疗后6 h后血乳酸水平以及血乳酸清除率,比较高、低血乳酸清除率组患者的呼吸衰竭、死亡及休克情况。结果死亡组和存活组患者的APACHE评分和初始动脉血乳酸水平比较差异不显著(P>0.05),但存活组6 h后动脉血乳酸清除率显著高于死亡组,差异具有统计学意义(P<0.05)。低血乳酸清除率组患者的呼吸衰竭率、死亡率及休克率均显著高于高血乳酸清除率组,差异具有统计学意义(P<0.05)。结论早期动脉血乳酸清除率在重症肺炎预后效果中具有较高的诊断价值,可以显著地反映出患者的炎性状态和疾病严重度,适合临床长期推广应用。
Objective To analyze the correlation of early arterial blood lactate clearance rate and the prognosis of severe pneumonia.Methods 42 cases of patients with severe pneumonia treated in our hospital from March 2014 to June 2015 were selected as research objects. All patients were divided into survival group(25 cases) and death group(17 cases), high blood lactate clearance group(23 cases)and low blood lactate clearance group(19 cases) according to different standards. The blood lactate levels before treatment and 6 h after treatment, blood lactate clearance rate of survival group and death group were observed and compared, and the incidence of respiratory failure, shock and death of high blood lactate clearance group and low blood lactate clearance group was also compared. Results The APACHE score and blood lactate level before treatment of survival group and death group had no statistical difference(P〉0.05), but the blood lactate level 6 h after treatment of survival group was significantly higher than that of death group, with statistically significant difference(P〈0.05). The incidence of respiratory failure, death and shock of low blood lactate clearance group was significantly higher than that of high blood lactate clearance group, with statistically significant difference(P〈0.05). Conclusions Early arterial blood lactate clearance rate has high diagnostic value in the prognosis of severe pneumonia, and can significantly reflect patient's inflammatory status and severity of disease, which is worthy of long-term clinical application.
