摘要
目的了解国内外胃癌相关研究动态、预测未来发展前景,为临床医师进行胃癌治疗、提高诊疗水平提供建议;为基础医学研究工作中的学科研究回顾、寻找新型研究领域、设计课题提供一定的帮助。方法综合运用文献计量学方法,复习2010年1月1日至2014年12月31日全球医学工作者对胃癌研究的状况,梳理胃癌高频词、主要发表论文等,运用统计学方法、共词聚类分析方法、战略坐标方法分析胃癌论文所展现的胃癌研究热点及热点变化。结果通过对高频主题词的词频分析、词篇聚类分析、共词矩阵分析、粘合力分析,发现近5年胃癌研究热点主要集中在有关幽门螺杆菌的研究、关于胃癌手术治疗的研究、胃癌联合化疗的研究以及肿瘤基因方向的研究。研究结果发现,幽门螺杆菌感染与胃癌的发生有密切相关性;手术治疗是治疗早期胃癌的主要手段,同时胃镜及内窥镜在胃癌治疗中发挥重要作用;在抗肿瘤联合化疗方面,主要成分为氧嗪酸钾和替加氟的口服化疗药替吉奥在胃癌治疗中发挥重要作用,替吉奥与顺铂或者奥沙利铂联合化疗治疗胃癌目前应用十分广泛。第三个内容为胃癌论文研究热点的变化,研究发现胃癌研究热点的变化主要在两个方面。第一个变化是化疗方案的改变;第二个变化时是基因层面的研究越来越受到重视,包括两个层面:一是胃癌靶向治疗逐渐成为研究热点,通过靶向药物治疗胃癌是未来的发展方向;未来胃癌治疗的另一个发展方向是通过DNA序列测定以及细胞的多样性,对患者进行个体化治疗。结论本研究通过文本数据挖掘,对比分析各年胃癌研究热点,把握胃癌研究动态及趋势,对实际医学科研及临床工作有一定指导意义。
Objective To investigate the recent research trends on gastric cancer all over the world,aimed to forecast future develop-ment trends,and to provide insights for clinicians to cure gastric cancer and to improve diagnosis and treatment level. We were also to help review related research,search for new research area and apply for project grants in the study of basic medicine. Methods Sci-entific research had provided gastric cancer research with the basic foundation and methods. Science literature was an important tool and method for researchers to spread ideas and knowledge,and as a material vehicle of scientific knowledge,science literature con-tained massive information and knowledge on gastric cancer research. Using bibliometrics methods, this paper concluded recent re-search trends on gastric cancer across the world and summarized the high-frequency words and main publications on gastric cancer. It also analyzed the popular research issues and its trends on gastric cancer using statistical methods,co-word clustering analysis and stra-tegic coordinate,etc. Results By analyzing the frequency of high frequency topic words,using word article clustering analysis,total word matrix analysis and adhesive force analysis,this paper found that the most popular research topics on gastric cancer in recent five years were those on helicobacter pylori,the surgical treatment of gastric cancer,the MDT of gastric cancer and the oncogene of gastric cancer. Research results indicated that the infection of helicobacter pylori closely correlates with gastric cancer,that surgical treatment was the main approach to cure early stage gastric cancer and that gastroscope and endoscope played important roles in gastric cancer treatment. Besides,on the antineoplastic combined chemotherapy protocols,the oral chemotherapeutic drugs,gimeraciland oteracil po-rassium capsules,whose main ingredients were oteracil potassium and tegafur,were very crucial in the treatment of gastric cancer. U-sing a combined chemotherapy of gimeraciland oteracil porassium capsules and cis-platinum or oxaliplatin to treat gastric cancer were widely used now. This paper found that the change of hot topics on gastric cancer had two kinds. The first one was the change of chem-otherapy regimens. The second change was that more and more importance is placed on gene research. One was that targeting therapy a-gainst gastric cancer are becoming popular research issues,and treating gastric cancer by targeted drugs was becoming a trend. Another trend was using DNA sequencing and the diversity of cells to give patients individual treatment. Conclusion This manuscript has an advanced option on the development and focus of stomach cancer. The authors synthetically use their professional knowledge to guaran-tee the veracity and facticity of the data. Conclusion of the research is based on the development tendency of stomach cancer,which may be helpful for basic medical sciences and clinical application.
