摘要
目的评价激光光纤介入内照射5-氨基酮戊酸盐酸盐-光动力疗法(OFI-ALA-PDT)和传统外照射5-氨基酮戊酸盐酸盐-光动力疗法(ALA-PDT)治疗兔耳痤疮模型的疗效,并对比两种方法的不良反应。方法参照美国皮肤病学会制定的Kligman法造模16只兔耳痤疮模型,采用煤焦油外涂和痤疮丙酸杆菌接种致兔耳发生痤疮样皮损,于皮损处外敷20%ALA后封包,每只兔耳沿长轴分为两部分,分别OFI-ALA-PDT和ALA-PDT疗法治疗,观察两种方案治疗兔耳痤疮样炎性丘疹的疗效和不良反应。结果家兔耳廓涂煤焦油2周后肉眼可见兔耳毛囊开口粉刺样发疹、接种痤疮丙酸杆菌处有痤疮样炎性丘疹,组织病理检查示其病变与人类痤疮炎性丘疹相似。治疗后第2、5、10d,肉眼观察光纤介入内照射组较传统外照射组皮疹消退快;治疗后第30d,肉眼观察两组疗效相似。在治疗后各个时间点肉眼观察,光纤介入内照射组副反应明显较传统外照射组少。结论 OFI-ALA-PDT疗法治疗兔耳痤疮模型可取得同传统外照射ALA-PDT疗法类似的疗效,且有更高的安全性和耐受性。
Objective To evaluate the efficacy of the laser fiber intervention-5-aminolevulinic acid-photodynamic therapy (OFI-ALA-PDT) and the traditional 5-aminolevuliuie acid-photodynamic therapy (ALA-PDT)to the rabbit ear ache model, and compare the side effects of the two therapies. Methods With reference to Kligman method made by American Academy of Der- matology, we molded 16 rabbit ear acne models, smeared coal tar outside and injected propionibacterium aches to result in acne- iform eruptions on rabbit's ears, and then encapsulated after compressing 20% ALA outside of the lesional skin. Every rabbit ear was divided into two parts, and was treat by OFI-ALA-PDT and ALA-PDT respectively. Observe the efficacy and side effects of the two therapies to treat the acneiform inflammatory papule of the rabbit ear. Results Comedonicus papules at the opening of the hair follicle and acneiform inflammatory papule at the site injected by propionibacterium aenes of the rabbit ears were visible to naked eye, after the ears were smeared by coal tar for two years. Histopathologic examination shows that the lesions were similar to acne- iform inflammatory papule of human. The second ,fifth and tenth day after the treatment, the decrease of the rash of OFI-ALA-PDT group was quicker than that of ALA-PDT group with naked-eye observation. The 30 th day after the treatment, the efficacy of the two groups were similar with naked-eye observation. The side effect of OFI-ALA-PDT group was obvious less than that of ALA-PDT group at every time points after treatment when observing with naked eye. Conclusion OFI-ALA-PDT and ALA-PDT have similar efficacy to the rabbit ear acne model, and the former has better safety and tolerant
